5.1. The subject of the contract is the supply of general-use medicines (necessary for the proper functioning of the Hospital's Departments and Clinics) 5.2. Detailed description of the subject of the contract, description of the requirements of the Purchaser in terms of implementation and acceptance specify: 5.3.1. Description of the subject of the contract - assortment-price form - Annex No. 1 to SWZ Package No. 1 - Darbepoetin alfa Package No. 2 - Thiamine hydrochloride Package No. 3 - Supplementary drugs for the set for shock 5.3.2. Draft contractual clauses - Annex No. 2 to SWZ. 5.4. The assortment-price form is part of the offer and shall not be supplemented. Failure to submit the assortment-price form will have negative consequences for the Contractor in the form of rejection of the offer on the basis of art. 226.1.5 of the Pzp Act. 5.5. The description of the subject of the contract and the draft contractual clauses should be read together with any changes to the content of the SWZ, resulting from, for example, responses to questions from Contractors. Other information is included in Section V of the SWZ.
