1) The subject of the procurement is group life insurance for employees. 2) Detailed specification of the subject of the procurement is in Section II SWZ and Section IV SWZ. 3) In accordance with the provisions of 438 of the Act in connection with art. 95 para. 1 of the Act, the requirements regarding the obligation to employ workers are specified in Section II SWZ and Section IV SWZ. 4) The Contracting Authority requires the Contractor to indicate in the offer, which parts of the contract the Contractor intends to subcontract to Subcontractors and to provide the names of the Subcontractors, if they are already known. 5) The Contracting Authority does not reserve the right to personal performance by the Contractor of key tasks related to the services. 6) Name and code of the Common Procurement Vocabulary: Code: 66511000-5 Life insurance services. 7) The Contracting Authority does not require the submission of subject-related evidence with the offer. 8) Acting under art. 133 para. 3 of the Act, the Contracting Authority establishes requirements regarding the confidentiality of information provided to Contractors during the procedure. The following parts of the SWZ are confidential: Section II SWZ – Description of the subject of the procurement. Section IV SWZ – Draft contractual provisions. 9) In accordance with art. 133 para. 3 of the Act, the Contracting Authority does not make the contents mentioned in point 8 publicly available on the website due to the protection of the confidential nature of the information contained in the SWZ. The files named “Section II SWZ – Description of the subject of the procurement” and “Section IV SWZ – Draft contractual provisions” placed on the procurement platform have been password-protected. In order to protect the confidential nature of the Confidential Information provided during this procedure, the Contracting Authority reserves the right that the password for the confidential information mentioned above will be provided after the Contractor submits a confidentiality statement signed with a qualified electronic signature. It is required to submit a statement by persons authorized to represent the Contractor (this authorization must result from the registry documents or a power of attorney attached to the submitted statement) and by persons who will participate in the procedure. The templates for the confidentiality statements are Annexes No. 3.1 and 3.2 to Section III SWZ. The Contracting Authority informs that letters with explanations of the contents of the SWZ regarding the confidential part of the procurement (if any are submitted during the conduct of the procedure) will also be published in an encrypted form – to decrypt it, the above password will be used (this password will be used throughout the conduct of the entire procedure). Statements should be submitted via the Procurement Platform, tab "Questions/Information". After their receipt and verification, the password for decrypting the documents will be sent to the e-mail address indicated in the information about the transmission of the password for decrypting the documents. Note: If the persons who will participate in the procedure do not have qualified electronic signatures, the Contracting Authority allows these statements to be signed manually by the persons submitting the statements, scanned, confirmed for conformity with the original by persons authorized to submit statements of will and knowledge in the name of the Contractor and in such form submitted to the Contracting Authority via the Procurement Platform.
