1. The subject of the contract is the supply of accessories, reagents and tests for the Department of Pathomorphological Diagnostics, along with the rental of the relevant equipment, hereinafter referred to as the "goods" for the needs of the Independent Public Healthcare Institution of the Voivodeship Hospital in Białystok, in the quantities, assortment and parameters specified in Annex No. 2 to the SWZ – Detailed Offer Form. 2. The description of the subject of the contract and the conditions for the execution of the contract are contained in the annexes: a) Detailed Offer Form [description of the subject of the contract for individual packages (parts)] – Annex No. 2 to the SWZ; b) Contract drafts (conditions for the execution of the contract): – Annex No. 7A to the SWZ (Packages No. 1-2); – Annex No. 7B to the SWZ (Packages No. 3-4).
