Poland – Construction work – Modernizacja obiektu sportowego przy ul. Lumumby 22/26 w Łodzi użytkowanego przez Akademicki Związek Sportowy Organizację Środowiskową – (Etap I) budowa zadaszonej trybuny z zapleczem

The subject of this tender is the construction of a grandstand for an athletics stadium with a covered seating area for 1,205 spectators and 9 people with disabilities (with the possibility of increasing the number of seats to 1,345) along with the backstage area located under the grandstand, as well as the construction of communication routes along with the remaining development of the area, including elements of small architecture and technical infrastructure, as well as the planting of new greenery. A detailed description of the subject of the tender can be found in Annex 1 to the SWZ and in the contract template in Annex 7 to the SWZ. (Numbering according to the SWZ): 40.1 The Contracting Authority does not envisage: 40.1.1. the conclusion of a framework agreement; 40.1.2. the submission of variant and partial offers; 40.1.3. settlement in foreign currencies; 40.1.4. an electronic auction; 40.1.5. the reimbursement of participation costs; 40.1.6. the requirement or possibility of submitting offers in the form of electronic catalogues or attaching electronic catalogues to the offer; 40.1.7. the award of the contract under the right of option on the basis of Article 441 of the Pzp Act. The offer consists of the following documents: 15.1 Offer Form – prepared according to the template - Annex 2 to the SWZ (signed with a qualified electronic signature). 15.2 Price Form – prepared according to the template - Annex 2a to the SWZ (signed with a qualified electronic signature). 15.3 Contractor's Declaration regarding the grounds for exclusion under Article 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions affecting the situation in Ukraine and Article 7(1) of the Act on Special Solutions in the Field of Counteracting the Support of Aggression against Ukraine and Serving to Protect National Security, submitted pursuant to Article 125(1) of the Pzp Act, in accordance with the template constituting Annex 5a to the SWZ (signed with a qualified electronic signature). In the case of contractors jointly applying for the contract, this declaration is submitted by each of these Contractors (this also applies to partners of a civil law partnership). 15.4 Declaration of the entity providing resources regarding the grounds for exclusion under Article 5k of Regulation 833/2014 concerning restrictive measures in connection with Russia's destabilising actions affecting the situation in Ukraine and Article 7(1) of the Act on Special Solutions in the Field of Counteracting the Support of Aggression against Ukraine and Serving to Protect National Security, submitted pursuant to Article 125(5) of the Pzp Act, in accordance with the template constituting Annex 5b to the SWZ (if applicable) (signed with a qualified electronic signature). 15.5 Power of attorney/powers of attorney for the person/persons signing the offer, if the offer is signed by a proxy, (if applicable). 15.6 In the case of an offer submitted by Contractors jointly applying for the award of the contract - a power of attorney to represent all Contractors jointly applying for the award of the contract, or a cooperation agreement, from which the relevant power of attorney will result. 15.7 Commitment of the entity providing its resources for the needs of the Contractor submitting the offer, (if applicable - project of the commitment Annex 8 to the SWZ) or another evidential instrument confirming that the Contractor, when implementing the contract, will have the necessary resources of these entities (signed with a qualified electronic signature). 15.8 Declaration of the division of obligations during the implementation of the contract, (if applicable) – project of the declaration Annex 9 to the SWZ (signed with a qualified electronic signature). 15.9 List of persons, appointed by the contractor to carry out the public contract, in particular those responsible for the provision of services, quality control or the supervision of construction works, together with information on their professional qualifications, authorisations, experience and education necessary to carry out the public contract, as well as the scope of the activities carried out by them and information on the basis for their disposal of these persons – Annex 6b to the SWZ (signed with a qualified electronic signature). The Contracting Authority requires the submission of a list of persons (Annex 6b to the SWZ) together with the offer due to the need to make a proper assessment of the offers in the non-price criterion – Experience of the Construction Manager (D). (In the event of failure to submit the List of Persons – Annex 6b to the SWZ together with the offer, the Contractor will receive 0 points in the offer assessment criterion Experience of the Construction Manager (D). The Contracting Authority will call upon the Contractor to submit the List of Persons (Annex 6b to the SWZ), in accordance with point 9.3 of the SWZ in order to confirm that the Contractor has met the professional capacity condition in accordance with point 8.1.4.2. 27 INFORMATION ABOUT THE FORESEEN CONTRACTS, REFERRED TO IN ART. 214(1) POINT 7 OF THE PZP ACT, IF THE CONTRACTING AUTHORITY FORESEES THE AWARD OF SUCH CONTRACTS The Contracting Authority envisages the possibility of awarding a contract consisting of the repetition of similar construction works on the basis of Article 214(1) point 7 of the Pzp Act for the amount of 10,626,339.21 PLN net plus the applicable VAT. The scope of the works includes the execution of: general construction works, site preparation for construction, earthworks, shaping of green areas, construction works in the scope of erecting complete construction objects or their parts, as well as works in the scope of land and water engineering, construction works in the scope of building power lines, electrical installations, road works, installation of road lighting, construction and auxiliary works in the scope of telephone lines and communication routes, water and sanitary installation works, works in the scope of surfaces, with the exception of roads, geological services, soil investigation services, horticultural services, supply of equipment. The implementation of the contract consisting of the repetition of similar construction works on the basis of Article 214(1) point 7 of the Pzp Act is planned on plots 43/49 and 43/52, 43/50, 43/48, 43/9, 43/41 and 43/5 within S-3. c.d in the SWZ.
