III. Description of the subject matter of the contract: 1. The subject of the contract is: „Sequential delivery of single-use medical devices” Main CPV code – 33100000-1 Medical equipment 2. The detailed description of the subject matter of the contract includes the Price Form - Annex No. 2 to SWZ. 3. The contract is divided into the following parts: Part No. 1: SMALL SINGLE-USE EQUIPMENT Part No. 2: TRACHEAL TUBES AND KIT FOR CRICOTHYROTOMY Part No. 3: TACTICAL EQUIPMENT Part No. 4: RESPIRATORY FILTERS AND ACCESSORIES FOR RESUSCITATORS Part No. 5: SINGLE-USE RESUSCITATORS Part No. 6: MEDICAL GLOVES Part No. 7: BLADES FOR VIDEOLARYNGOSCOPE VS-10 Part No. 8: BLADES FOR VIDEOLARYNGOSCOPE VL 3D Part No. 9: BLADES FOR VIDEOLARYNGOSCOPE TDC-K UET-C Series Part No. 10: SPECIALIST MEDICAL PRODUCTS Part No. 11: NEONATAL RESUSCITATION EQUIPMENT Part No. 12: N/FABIAN SET EQUIPMENT Part No. 13: N/STEPHEN SET EQUIPMENT Part No. 14: SINGLE-USE METAL INSTRUMENTS Part No. 15: DRESSING MATERIALS Part No. 16: DRAPES Part No. 17: VENTILATION DRAPES 4. The contracting authority allows the submission of partial offers for one or any number of parts. 5. The contracting authority does not allow the submission of alternative offers. 6. The contracting authority does not envisage the awarding of contracts, as referred to in Article 241(1)(7) and (8) of the Pzp. 7. The contracting authority does not require the contract to be performed by sheltered workshops, social cooperatives or other contractors, as referred to in Article 94(1) of the Pzp. 8. The contracting authority does not envisage the requirements, as referred to in Articles 95 and 96 of the Pzp. 9. The contracting authority does not envisage a site visit. 10. If the contracting authority has specified trademarks, patents or the origin, sources or a particular process which characterises the products or services supplied by a particular contractor, equivalent solutions may be offered, provided that they meet the same minimum technical, quality and functional parameters described in the Description of the subject matter of the contract. 11. The contracting authority reserves the right to increase (by a maximum of 30%) the quantity of infusion fluids ordered under the individual positions of fluids covered by the subject of the contract and to purchase them at the unit prices specified in Annex No. 2 to the contract, provided that the maximum contract value specified in § 5(7) of this contract is not exceeded.
