Poland – Medical equipments – „Zakup aparatury i wyposażenia medycznego” w ramach umowy nr DOI/SK/85112/6220/305/643 na udzielenie dotacji celowej na dofinansowanie inwestycji pn. „Unowocześnienie i doposażenie Kliniki Gastroenterologii i Chorób Wewnętrznych” na potrzeby Kliniki Gastroenterologii dla SP ZOZ CSK UM w Łodzi

1. The subject of the contract is the implementation of the task entitled: “Purchase of medical equipment and supplies” within the framework of the agreement no. DOI/SK/85112/6220/305/643 for the grant of a specific subsidy to co-finance the investment entitled “Modernization and Equipment of the Clinic of Gastroenterology and Internal Diseases” for the needs of the Clinic of Gastroenterology for SP ZOZ CSK UM in Łódź. 2. In particular, the subject of the contract includes: pulse oximeters, fumigators, suction devices, transport trolleys, refrigerators-freezers, beds with electric height adjustment, chairs for blood collection, ultrasound machines, vital signs monitors, EKG machines, surgical lamps, disinfecting washers, macerators for diapers, devices for heating infusion fluids, infusion pumps, trolleys, mortuary trolleys, hospital beds, defibrillators, cardiomonitors, etc. in the quantities specified in Annex No. 2 to the SWZ. Due to the extensive description - a detailed description of the subject of the contract, details of the implementation of the subject of the contract, etc., are contained in Annex No. 2 to the SWZ – Description of the Subject of the Contract – Technical Parameters (hereinafter referred to as “OPZ”). 3. The Contractor is obliged to attach to the OFFER, in particular, Annex No. 2 to the SWZ - OPZ - Technical Parameters completed in accordance with the requirements of the Buyer. 4. The failure to submit Annex No. 2 to the SWZ constitutes an inability to assess the offer and thus a basis for rejecting the offer from the procedure. 5. The delivered subject of the contract must have an operating manual in Polish (and if required), be accompanied by the appropriate documents, as mentioned in the SWZ, must be covered by at least a warranty period - specified for a given device, equipment, specified in Annex No. 2 to the SWZ. In case of doubt as to whether the offered equipment, devices meet the technical parameters, the Buyer is entitled to request documents confirming the technical parameters of the offered equipment, devices, e.g. catalog cards, certificates, etc. 7. The equipment delivered to the Buyer must meet the following conditions: a) the delivered subject of the contract must be factory new, non-second-hand and free from encumbrances by third parties, b) have a warranty period not shorter than that specified for individual devices, equipment in Annex No. 2 to the SWZ, and if not specified in the OPZ, contract documents, it is 24 months. c) the offered devices on the date of submission of the offers may not be intended by the manufacturer for withdrawal from production or sale, d) All delivered devices must come from a legal source. 8. NOTE: Whenever the documentation specifies a brand or origin of the product, name, standard, certificate, device, it should be assumed that after each name is placed the word “or equivalent”, i.e. that the built-in materials, devices, etc. will have (be characterized by) all the parameters no worse than those described in this documentation, for a given OPZ position. 9. If specific technical solutions, standards, certificates are indicated in the description of the subject of the contract, the use of equivalent solutions in terms of their features and parameters – specified for a given subject of the contract position, and all possible trade names of devices and products used in the description of the subject of the contract should be treated as standard definitions, and not as specific trade names of devices, products used in this documentation. The obligation to prove equivalence lies with the Contractor. The Buyer allows for the submission of partial offers. The contract is divided into 15 contract parts. The Contractor may submit an offer for any number of contract parts.
