1. The subject of the order is the delivery of single-use and reusable equipment for the hemodynamic laboratory and electrophysiology laboratory for Specjalistyczny Szpital Miejski im. M. Kopernika in Torun, divided into 15 parts. 2. The detailed description of the subject of the order is contained in Annex No. 1 to the SWZ, which also constitutes the assortment-price form. The offered subject of the order must be approved for sale and use in accordance with the Act of 7 April 2022 on medical devices, Regulation (EU) 2017/745 on medical devices, and Regulation (EU) 2017/746 on in vitro medical devices. 3. The ordering party does not limit the number of order parts submitted by one Contractor. One Contractor may submit a bid for the entire order or for any number of its parts. The bid must include all the assortment items listed in Annex No. 1, appropriately for each of the parts. Bids for incomplete parts are not allowed. 4. Order parts: 1) Part 1 – Stents II, 2) Part 2 – Stents IV, 3) Part 3 – Stents V, 4) Part 4 – Blood pressure monitoring set, 5) Part 5 – Hydrophilic conductors, 6) Part 6 – Test ACT+ for Hemochrom Signature Elite, 7) Part 7 – Conducting catheters II, 8) Part 8 – Basic and high-pressure balloon catheters I, 9) Part 9 – Basic and high-pressure balloon catheters II, 10) Part 10 – NC II balloon catheters, 11) Part 11 – Heart pacemakers I, 12) Part 12 – Heart pacemakers II, 13) Part 13 – Cardioverter I, 14) Part 14 – Cardioverter II, 15) Part 15 – Cardioverter III.
