Description

The subject of the contract is the provision of the service of collection, transport, and disposal of medical waste from the facilities of the University Clinical Centre (UCK) located at: 1) ul. Dębinki 7, Gdańsk, 2) ul. Smoluchowskiego 17 (CMN and CMI), Gdańsk, 3) Al. Zwycięstwa 30, Gdańsk, 4) ul. Jana Kilińskiego 4 (METROPOLIA), Gdańsk, 5) UCMMiT at ul. Powstania Styczniowego 9b, Gdynia. 2. The contracting authority allows the submission of partial offers. The contracting authority understands the term "partial offer" to mean individual packages as specified below: 1) Package 1 - Collection and disposal of infectious and hazardous medical waste from the following locations and in estimated quantities: a) ul. Dębinki 7, Gdańsk, in an estimated quantity of - 170 Mg/36 months; one room for medical waste; b) ul. Smoluchowskiego 17, Gdańsk (CMN and CMI), in an estimated quantity of – 3 200 Mg/36 months; two rooms for medical waste (waste from the room to the vehicle must be transported in a container); c) Al. Zwycięstwa 30, Gdańsk, in an estimated quantity of - 176 Mg/36 months; one room for medical waste; 2) Package 2 – Collection and disposal of infectious and hazardous medical waste from the following locations and in estimated quantities: a) ul. Jana Kilińskiego 4, Gdańsk (METROPOLIA), in an estimated quantity of - 7 Mg/36 months; one room for medical waste; b) ul. Powstania Styczniowego 9b, Gdynia (UCMMiT), in an estimated quantity of 3 Mg/36 months; one room for medical waste. The contractor may submit an offer for one or both packages. 3. The estimated quantities specified in point 2 may change, depending on the contracting authority's demand. 4. In the scope of each of the Packages, the contractor undertakes to collect and dispose of medical waste by incineration of medical waste with the following codes: 1) 18 01 01 - surgical and examination instruments and their residues (excluding 18 01 03*); 2) 18 01 02* - body parts and organs and containers for blood and preservatives used for its storage (excluding 18 01 03*); 3) 18 01 03* - other waste that contains live pathogenic microorganisms or their toxins and other forms capable of transferring genetic material, about which it is known or for which there are reliable grounds to assume that they cause diseases in humans and animals, excluding 18 01 80 and 18 01 82; 4) 18 01 06* - chemicals, including chemical reagents containing hazardous substances; 5) 18 01 08* - cytotoxic and cytostatic drugs; 6) 18 01 09 - other drugs than those mentioned in 18 01 08*; 7) 18 01 82* - remnants from the feeding of patients from infectious wards. 5. In cases where the contracting authority has used in the description of the subject of the contract designations of standards, technical assessments, technical specifications and technical reference systems, as referred to in Article 101(1)(2) and (3) of the Pzp, equivalent solutions described in the SWZ are allowed. Each time a standard is indicated in this SWZ or in the annexes to the SWZ, it should be understood that the formulation "or equivalent" is used in relation to it. In cases where the description of the subject of the contract refers to standards, technical assessments, technical specifications and technical reference systems, as referred to in (1)(2) and (3), the contracting authority may not reject an offer solely on the grounds that the offered services are not in accordance with the standards, technical assessments, technical specifications and technical reference systems to which the description of the subject of the contract refers, provided that the contractor proves in the offer, in particular by means of evidential means referred to in Articles 104-107, that the proposed solutions meet the requirements specified in the description of the subject of the contract to an equivalent extent. 6. A detailed description is contained in Section IV of the SWZ.
