The subject of the procurement is: "Service of conducting group training for medical personnel as part of the project 'Coordinated Care in Primary Health Care'". 2. The procurement is related to the implementation of project task no. 8, as part of the project 'Coordinated Care in Primary Health Care' (project number: FERS.04.15-IP.07-003/23). 3. The contracting authority allows for the submission of partial offers. The procurement is divided into four separate parts: Part A covers: woj. zachodniopomorskie, pomorskie, kujawsko-pomorskie, wielkopolskie, Part B covers: woj. warmińsko-mazurskie, podlaskie, łódzkie, mazowieckie, Part C covers: woj. lubuskie, dolnośląskie, opolskie, śląskie, Part D covers: woj. świętokrzyskie, małopolskie, lubelskie, podkarpackie. NOTICE: THE CONTRACTOR MAY SUBMIT AN OFFER FOR A MAXIMUM OF TWO PARTS. 4. The description of the subject of the procurement is contained in Annex No. 1 to the Specification. 5. The method of implementing the procurement is specified in the contract project constituting Annex No. 2 to the Specification. 6. The contracting authority does not envisage awarding contracts under Article 305 in connection with Article 214(1)(7) and (8) of the Pzp Act.
