1. The subject of the contract is: SUPPLY OF MEDICAL PRODUCTS AND SINGLE-USE EQUIPMENT FOR PROCEDURES IN THE HEMODYNAMICS LABORATORY for the Wojewódzki Szpital Specjalistyczny im. Najświętszej Maryi Panny in Częstochowa. 2. The subject of the contract is described in the itemised price forms, which constitute Annex 2 to the SWZ. Other conditions relating to the execution of the contract are specified in Annex 6 to the SWZ, constituting the draft contractual terms. 3. The subject of the contract includes 18 lots. The contracting authority allows partial tenders to be submitted. The term 'partial tender' refers to individual lots as specified in Annex 2 to the SWZ. The contractor may submit a tender for any of the lots or for the entire subject of the contract.
