1. Successive deliveries of medicines for the National Institute of Oncology named after Maria Skłodowska-Curie, National Research Institute, Gliwice Branch

2. The contracting authority allows for the submission of partial offers, and therefore the subject of the order is divided into 3 separate tasks, namely: 1) Oral diet intended - for preoperative treatment of patients 2) Pemigatinib 3) Various 2

3. The description of the subject of the order and the conditions for the implementation of the order are contained in the annexes: 1) Assortment-price specification – Annex No. 2.1-2.3 to SWZ, 2) Draft contractual provisions, which will be introduced into the content of the contract – Annex No. 4 to SWZ.

4. The contracting authority did not conduct preliminary market consultations.

5. The contracting authority does not foresee additional orders.

6. The contracting authority does not foresee an electronic auction, additional orders.

7. The contracting authority does not foresee the requirement: 1) for the Contractor to conduct a local visit or 2) for the Contractor to check the documents necessary for the implementation of the order available on site at the contracting authority.

8. Information on the joint tendering of Contractors for the award of the order is contained in Section IX of the SWZ.

9. The validity period of the offer is: 120 days. The validity period of the offer begins with the expiry of the deadline for the submission of offers. This day is the first day of the validity period of the offer. The above means that the validity period of the offer expires on 28.01.2027.

10. The contracting authority has included the information clause resulting from the content of Article 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, in the SWZ.
