Poland – Medical practice services – Usługi i świadczenia zdrowotne w zakresie medycyny pracy (zamówienie z podziałem na 4 części)

The subject of the contract is the provision of healthcare services and medical services in the field of occupational medicine and other healthcare services, including the performance of diagnostic tests, specialist examinations, specialist consultations, medical opinions in the field of occupational medicine, a doctor's participation in the work of the OSH commission, workplace assessments along with occupational risk assessment, issuing referrals to the RWKL with a medical opinion, issuing referrals for therapeutic-prophylactic appointments, issuing medical opinions on fitness to drive vehicles, issuing medical opinions on physical and mental fitness to perform the duties of a physical protection worker, and other medical services and healthcare for professional soldiers and employees of the Ministry of National Defence, both those employed and newly recruited, at the 34 WOG in Rzeszów and in military units and institutions under its supply. 2. The subject of the contract has been divided into 4 (four) parts as follows: 1) Part No. 1: Healthcare services and medical services in the field of occupational medicine and other medical services in the locality of Rzeszów, 2) Part No. 2: Healthcare services and medical services in the field of occupational medicine and other medical services in the locality of Nisko/Stalowa Wola, 3) Part No. 3: Healthcare services and medical services in the field of occupational medicine and other medical services in the locality of Jarosław/Przeworsk, 4) Part No. 4: Healthcare services and medical services in the field of occupational medicine and other medical services in the locality of Przemyśl. 3. The detailed description of the subject of the contract is Annex No. 5 to the SWZ (applies to each part of the contract). 4. The detailed quantitative specification is the Price Offer Calculation Form: 1) Annex No. 1A to the SWZ – in the scope of Part No. 1; 2) Annex No. 1B to the SWZ – in the scope of Part No. 2; 3) Annex No. 1C to the SWZ – in the scope of Part No. 3; 4) Annex No. 1D to the SWZ – in the scope of Part No. 4. 5. The draft provisions of the contract regarding the public contract, which will be introduced into the content of this contract, are Annex No. 4 to the SWZ – in the scope of Parts No. 1, 2, 3, 4 – also includes the supplementation of the content of the description of the subject of the contract.
