The subject of the contract are successive deliveries of sets for automatic peritoneal dialysis of type ADO and sets for continuous ambulatory peritoneal dialysis of type CADO to the specified home address of the patient or to the Hospital Pharmacy Warehouse. The contract includes 4 parts (tasks). A detailed description of the subject of the contract, its size, scope and method of implementation is contained in the specification of the contract conditions (hereinafter referred to as SWZ) and its annexes.
