The subject of the contract is "Supply of equipment, tools and other medical equipment for the Clinic of Gynaecological Oncology and Pathology of Pregnancy and Gynaecology SP ZOZ Central Hospital Clinical UM in Łódź" 1.1. The detailed description and scope of the contract subject is specified in Annex No. 2 to the Specification of Contract Conditions (SWZ). 1.2. The Purchaser has divided the contract into individual parts and allows for the possibility of submitting partial offers - Packages: No. 1 – No. 7 The offered contract subject must: meet the requirements specified by the Purchaser in the Specification of Contract Conditions, in particular the conditions specified in the description of the contract subject contained in Annex No. 2 SWZ. 1.3. Common Procurement Vocabulary (CPV): 33140000-3 Medical materials; 33169000-2 Surgical instruments 2. Each of the Contractors may submit an offer for one or more parts, provided that all items in the assortment-price form - Annex No. 2 (in the chosen package or packages) are priced and the final amount for the entire contract subject – package is stated. Failure to price a given item in the package will result in the offer being rejected. Failure to meet any of the boundary conditions specified in the aforementioned Annexes will result in the offer being rejected. 3. The Purchaser undertakes to implement the contract subject to the minimum value of 50% of the gross contract value. 4. Contract performance period: within 24 months from the date of conclusion of the contract.
