The subject of the contract is the purchase and delivery of single-use equipment necessary for performing vascular surgery procedures. 2. The subject of the contract has been divided into 5 parts (5 lots). In each case, if the SWZ does not indicate to which part of the contract a given entry refers, it should be assumed that the entry relates to both parts of the contract. 1) Part 1 (Lot 1) purchase and delivery of single-use equipment necessary for performing vascular surgery procedures – endovascular procedures a) Scope: The subject of the contract includes the purchase and delivery of various types of stents, catheters, distal protection systems, procedural guides, systems for closing vascular accesses, vascular plugs, bioresorbable scaffolds and others. b) Common Procurement Vocabulary (CPV): Main CPV code: 33140000-3 – Medical materials. Additional CPV codes: 33141620-2 – Medical kits 33190000-8 – Various medical devices and products c) The detailed description of the subject of the contract for Part 1 (Lot 1) is defined in Annex 1A to the SWZ – Description of the subject of the contract for Part 1 (which also constitutes a price form calculating the price of the offer for Part 1). In the scope of the detailed description of the subject of the contract, the provisions of the Draft Contract Terms for the Public Contract constitute Annex 6 to the SWZ (common to all parts). 2) Part 2 (Lot 2) purchase and delivery of single-use equipment necessary for performing vascular surgery procedures – stent grafts and others a) Scope: The subject of the contract includes the purchase and delivery of various types of stent grafts, subrenal extensions, vascular prostheses and others. b) Common Procurement Vocabulary (CPV): Main CPV code: 33140000-3 – Medical materials. Additional CPV codes: 33141620-2 – Medical kits 33190000-8 – Various medical devices and products c) The detailed description of the subject of the contract for Part 2 (Lot 2) is defined in Annex 1B to the SWZ – Description of the subject of the contract for Part 2 (which also constitutes a price form calculating the price of the offer for Part 2). In the scope of the detailed description of the subject of the contract, the provisions of the Draft Contract Terms for the Public Contract constitute Annex 6 to the SWZ (common to all parts). 3) Part 3 (Lot 3) Purchase and delivery of Vascular Surgery Instrumentarium a) Scope: The subject of the contract includes the purchase and delivery of various types of special guides, catheters, high-pressure syringes, stents and others. b) Common Procurement Vocabulary (CPV): Main CPV code: 33140000-3 – Medical materials. In the scope of the detailed description of the subject of the contract, the provisions of the Draft Contract Terms for the Public Contract constitute Annex 6 to the SWZ (common to all parts). 5) Part 5 (Lot 5) Purchase and delivery of Vascular Surgery Instrumentarium a) Scope: The subject of the contract includes the purchase and delivery of various types of special stent grafts, catheters, vascular jackets, guides, angiographic needles, connecting wires and others. b) Common Procurement Vocabulary (CPV): Main CPV code: 33140000-3 – Medical materials. Additional CPV codes: 33141620-2 – Medical kits 33190000-8 – Various medical devices and products c) The detailed description of the subject of the contract for Part 5 (Lot 5) is defined in Annex 1E to the SWZ – Description of the subject of the contract for Part 5 (which also constitutes a price form calculating the price of the offer for Part 5). In the scope of the detailed description of the subject of the contract, the provisions of the Draft Contract Terms for the Public Contract constitute Annex 6 to the SWZ (common to all parts). 3. Offers can be submitted in relation to: all parts. The purchaser does not specify the maximum number of contract parts to which a single Contractor may be awarded a contract. However, the purchaser requires that the Contractor submits offers for the entire range of products in a given lot. 4. It is absolutely necessary that the offered medical products, which are the subject of the contract, meet the requirements specified in the Act of 7 April 2022 on medical devices (i.e. Journal of Laws of 2024, item 1620) and have a Declaration of Conformity issued by the manufacturer. Note: The Contractor whose offer is selected as the most advantageous is obliged to present the aforementioned documents to the Purchaser upon each request within 3 working days from the date of receipt of the written request.
