The subject of the contract is the supply of “Various medical devices and products – single-use equipment” for the Independent Public Health Care Unit in Brzesko, at 68 Kościuszki Street. The subject of the contract has been divided into 28 tasks. A detailed description of the subject of the contract is provided in the Detailed Offer Form, which is Annex 1 to this SWZ, and the draft contractual provisions, which are Annex 5 to the SWZ.
