The subject of the contract is: the purchase of medical equipment and devices. A detailed description of the subject of the contract in terms of the required parameters is specified in the annexes no. 2.1 – 2.5 to the SWZ – Description of the Subject of the Contract (contract item/package/task no. 1 – 5) – also referred to as “Annex no. 2 to the SWZ”. The contracting authority allows for the submission of partial offers (5. – tasks/parts of the contract), in accordance with the division specified in Annex no. 2 to the SWZ, constituting the form of the Description of the Subject of the Contract: 1) Task no. 1 – Surgical diathermy with accessories; 2) Task no. 2 – Endoscopy unit equipment; 3) Task no. 3 – Transport stretcher; 4) Task no. 4 – Ultrasound machine; 5) Task no. 5 – Patient warming system. The contracting authority does not allow for the submission of alternative offers or in the form of electronic catalogues. The subject of the contract includes all costs related to the execution of the contract, including costs resulting from the requirements specified in the SWZ, Annex no. 2 to the SWZ and Annex no. 4 to the SWZ.
