Poland – Analysers – Dzierżawa analizatorów wraz z dostawą odczynników oraz dostawa testów do Laboratorium Diagnostycznego szpitala Olmedica w Olecku Sp. z o. o.

1. The subject of the procurement is: „Lease of analysers along with the supply of reagents and the supply of tests for the Diagnostic Laboratory of Olmedica Hospital in Olecko Sp. z o. o.” 2. The detailed description of the subject of the procurement is defined in the description of the subject of the procurement constituting Annex No. 4 to the SWZ and in the contract template constituting Annex No. 2, 2a and 2b to the SWZ. 3. The estimated quantities specified in Annex No. 4 to the SWZ may be reduced, depending on the needs of the Buyer. The Buyer is not obligated to order the remaining unfulfilled part of the goods during the contract period due to reduced demand. 4. The subject of the procurement must meet the requirements specified in the Act of 07 April 2022 on medical devices (i.e. Journal of Laws 2024 item 1620 as amended), must have documents allowing for trade and use in Poland, in particular, must be marked with the CE mark, and if the conformity assessment was carried out under the supervision of a notified body, the identification number of the relevant body must be placed next to the CE mark. The Contractor, upon each request of the Buyer, will present certified copies of the permits (certificates) in accordance with the original, and if the subject of the procurement has not been classified as a medical device and, in accordance with European directives and the Act on medical devices, is not subject to declarations of conformity and is not subject to any entry in the register, the Contractor will submit the appropriate declarations to the Buyer when the Buyer so requires. 5. The Buyer requires that the offered products meet the requirements specified by the applicable law, have been admitted to trade, and possess the legally valid documents attesting to their admission for use in Poland.
