Poland – Medical consumables – Dostawa materiałów medycznych (sprzętu jednorazowego użytku) na potrzeby Szpitala Głogowskiego sp. z o.o. – powtórka.

The subject of the order is: 1. The subject of the order is the successive delivery of medical materials – single-use equipment for the needs of the Hospital Pharmacy for hospital wards. 2. The subject of the order is divided into 35 lots. The offer may cover the entire order or selected lots. An offer for its validity in lots no. 2, 4, 5, 6, 7, 8, 9, 10, 12, 13, 17, 19, 20, 22, 24, 25, 26, 27, 28, 29, 31, 34, 35 must be submitted for all items in the lot. The ordering party allows offers to be submitted for selected items within Lots no. 1, 3, 11, 14, 15, 16, 18, 21, 23, 30, 32, 33. 3. The subject of the order is described in detail in Appendix No. 1 to the SWZ, i.e. the Price Form (quantitative-assortment specification). 4. The ordering party allows the offer of products in different unit packages provided that the final quantity is identical to that specified by the ordering party in the Price Specification Form. If the total number of items is not divisible by the size of the package, the ordering party allows the pricing of products taking into account the quantity conversions accurate to two decimal places, after conversion in accordance with mathematical rules. 5. The ordering party allows partial offers to be submitted in the procedure, i.e. for any selected lots. 6. The contractor, after receiving information about the result of the procedure and before concluding the contract, undertakes to send electronically the file of attachment no. 1 in an editable format. 7. The ordering party reserves the right to purchase a smaller quantity of products. 8. During the implementation of the contract, the ordering party may make quantitative shifts between individual items of the assortment within the value of the contract in relation to each lot. 9. The offered subject of the order must meet the legal requirements allowing their use. They must have documents allowing their use in healthcare facilities in the Republic of Poland, in accordance with the requirements specified in the Act of 07 April 2022 on medical devices and the executive regulations to this Act, i.e. a declaration of conformity with the essential requirements CE and, if required by separate regulations – a certificate from a notified body, which participated in the assessment of the medical device, specified in the Act on medical devices - if not applicable, an appropriate declaration must be submitted with the offer. 10. The contractor ensures the validity of the documents allowing the supplied products to be placed on the market and used for the duration of the contract. 11. The required shelf life of the subject of the order - at least 12 months from the date of delivery. 12. It is required that each individual item of the offered assortment is equipped with a commercial label made in the Polish language, the collective packages must be equipped with the same label. In addition, the commercial label should contain at least the following information:  type and name of the product  number of items in the package  name of the manufacturer  code number, size  batch number  expiry date 13. The offered assortment must meet the requirements of the ordering party as stated in the description of the subject of the order and fully meet the functional requirements appropriate for these products in accordance with their intended use. 14. It is the Contractor's obligation to ensure the continuity of the supply of the assortment covered by the contract for the entire period of the contract. 15. Orders will be placed electronically, depending on the current needs of the ordering party. 16. The selected contractor undertakes, within the framework of the future contractual service, at its own risk, to deliver the subject of the order to the ordering party's headquarters, i.e. Hospital Pharmacy. The delivery of the ordered goods will take place on the basis of the order placed by the authorized representative of the ordering party, within a maximum of 2 working days. 17. In order to confirm the quality requirements in relation to the offered subject of the order, the contractor will attach to the offer informative materials: current catalogues, brochures, leaflets, etc. On the attached informative materials, the lot number and the lot item number must be clearly indicated. 18. The main subject of the order according to the Common Procurement Vocabulary (CPV): 33 14 00 00-3 medical materials 19. The ordering party does not allow the submission of variant offers. 20. The ordering party does not provide for: 19.1. supplementary orders, 19.2. framework agreement, 19.3. use of electronic auction, 19.4. establishment of a dynamic purchasing system. 19.5. granting advances on account of the performance of the order 20. The ordering party provides for the use of the right of option in the case of supplementary orders. The detailed scope and conditions for the execution of the order are specified by: Contract Project – Appendix no. 3 to the SWZ.
