M. Kopernika w Łodzi

The subject of this order is the supply of medical, laboratory, and food gases, the rental of a cryogenic tank with atmospheric vaporiser, and the rental of gas cylinders for the Voivodeship Multispecialist Oncology and Traumatology Centre named after M. Kopernik in Łódź. The order includes 8 lots. The offer may cover the entire order or selected lots. For its validity in a given lot, the offer must be submitted for all items. The buyer does not allow offers to be submitted for items within the lots. The subject matter has been described in detail in the annex to this SWZ, namely: the “product and price list” – Annex No. 2 and in the contract template – Annex No. 7. The content of the offer contained in Annex No. 2 must unambiguously indicate which product the Contractor is offering, by specifying data allowing for the identification of the offered assortment, i.e.: trade name, model/catalogue number (if marked), manufacturer's name, etc. The data identifying the offered assortment must be identical to the data that the Contractor will use when issuing the invoice. When specifying the offered product in Annex No. 2 to the offer using, among other things, the catalogue number, the Contractor must provide the complete number/symbol assigned to the given product without using designations such as “xxx”, “***” etc.
