Poland – Antiseptics and disinfectants – Dostawa preparatów do dezynfekcji powierzchni i narzędzi na potrzeby Samodzielnego Publicznego Zakładu Opieki Zdrowotnej w Lubartowie

The subject of the procurement is the successive delivery of disinfectant preparations and materials for the needs of the Independent Public Health Care Institution in Lubartów, divided into three parts: 1) Part 1 – Surface and instrument disinfectant preparations; 2) Part 2 – Instrument disinfectant preparations; 3) Part 3 – Instrument and small surface disinfectant preparations. 2. The detailed description of the subject of the procurement and the assortment-price forms are contained in the respective annexes 1a, 1b and 1c to the SWZ. The requirements indicated in these annexes apply to the products identified in the offer, taking into account the principles of equivalence specified in this section. 3. Ordinary deliveries are made on the basis of partial orders. The maximum delivery time is 7 working days from the day following the day of receipt of the order. The term declared in the offer is assessed in accordance with section XX and is binding during the implementation of the contract. Working days are considered to be days from Monday to Friday, excluding legally non-working days. 4. The purchaser guarantees the implementation of the basic scope with a value of at least 80% of the initial gross basic value of the given part, in accordance with § 3, 7 of the contract template. The quantities specified in the assortment-price form constitute the maximum basic scope, subject to a separate option. 5. The purchaser provides the right of option in accordance with § 1, 3 of the contract template. The option covers additional deliveries of positions specified in the OPZ up to the initial maximum value of 30% of the initial gross basic value of the given part, taking into account the permitted price changes. The maximum quantities of positions covered by the option are specified in the option table added to the relevant annex 1a, 1b or 1c. The option can be implemented during the basic or additional period, with the total implementation period not exceeding 30 months from the conclusion of the contract.
