The subject of the contract is the supply of medicines, the detailed description, assortment and estimated quantities of which have been specified in the Price and Quantity Form, which is Annex 1 to the SWZ. The detailed description of the subject of the contract is described in the contract template, which is Annex 2 to the SWZ. Sections 2 to 8 cover medicines ordered under the medicinal programmes. Contractors may jointly apply for the award of the contract: 1) in the case of a civil partnership the Contracting Authority accepts that the Contractors are the partners of the civil partnership, whose participation in the procedure is treated as a joint application for the award of the contract within the meaning of Article 58(1) of the Pzp Act; 2) Contractors acting jointly appoint a representative to represent them in the procedure for the award of the contract or to represent them in the procedure and conclude the contract in the matter of the public contract; 3) The power of attorney to perform the acts referred to in point 2 should be in the form of a document confirming the appointment of the representative, signed by the representatives of all the remaining Contractors, who are entitled to represent them. The power of attorney must be attached to the tender; 4) Contractors jointly applying for the award of the contract shall bear joint and several liability for the performance of the contract; 5) The tender must be signed in such a way as to bind all contractors acting jointly; 6) All correspondence and settlements will be made with the contractor acting as the representative of the others (leader); 7) If a tender from contractors jointly applying for the award of the contract is selected, the Contracting Authority may, before concluding the contract in the matter of the public contract, request a copy of the agreement regulating the cooperation of those contractors; 8) Contractors jointly applying for the award of the contract submit the Tender Form jointly.
