1. The subject of the contract is the purchase of necessary equipment for the provision of services for children with cerebral palsy (CP), planned within the projects entitled “Utilisation of modern diagnostic technologies and robotised rehabilitation systems in the therapy of children with cerebral palsy in Mazowieckie Centrum Neuropsychiatrii Sp. z o.o. in Zagórze – edition III for RWS.” and “Utilisation of modern diagnostic technologies and robotised rehabilitation systems in the therapy of children with cerebral palsy in Mazowieckie Centrum Neuropsychiatrii Sp. z o.o. in Zagórze - edition III for RMR.”, co-financed by the European Social Fund Plus within Priority VII “European Funds for active integration and the development of social and health services in Mazovia” Action 8.5 Social and health services of the programme European Funds for Mazovia 2021-2027, including delivery and installation. 2. The contracting authority has divided the contract into parts - 3 parts. 1) Part no. 1 - purchase of an EMG (electromyographic) system for diagnosing needs in the field of neurorehabilitation; 2) Part no. 2 – purchase of a mobile electromyograph with triggered electrostimulation; 3) Part no. 3 - purchase of a minimum 34-channel system for examining the electrical activity of muscles (EMG). 3. The contractor may submit a tender for one or more than one part. 4. A detailed description of the subject of the contract is contained in the Forms of required technical parameters, respectively: 1) Annex no. 9 to SWZ – form of required technical parameters for part no. 1; 2) Annex no. 9a to SWZ – form of required technical parameters for part no. 2; 3) Annex no. 9b to SWZ – form of required technical parameters for part no. 3.
