The subject of the contract is: NEUROSURGICAL IMPLANTS for the Specialist Hospital of the Virgin Mary, Queen of Poland in Częstochowa. 2. The subject of the contract is described in the assortment-price forms, which are Annex No. 2 to the SWZ. The remaining conditions regarding the execution of the contract are specified in Annex No. 6 to the SWZ, which constitutes the draft contract clauses (PPU). 3. The contracting authority allows the submission of partial offers. Partial offer means individual parts determined in Annex No. 2 to the SWZ. The contractor may submit an offer for any parts or for the entire subject of the contract. 4.
