The subject of the order is governed by the provisions of the Act of 12 May 2011 on the refunding of medicines, special dietary foods and medical devices (Official Journal of 2011 No. 122, item 696, as amended). In the case of medicines, special dietary foods and medical devices covered by the refunding of prices, the prices offered and applied by the Contractor may not exceed the maximum prices determined on the basis of Article 9(1) of the Act of 12 May 2011 on the refunding of medicines, special dietary foods and medical devices. The unit packaging of the product must contain a visible CE marking, batch number, expiry date and single-use mark, with the size and quantity of items in the packaging clearly visible. The detailed description of the subject of the order is specified in the assortment-price form - Annex 2 to SWZ. The Purchaser allows for other packaging sizes, converted to full packages (rounded up). The Contractor is obliged to implement the order on the principles and conditions described in the contract template constituting Annex 4 to SWZ.
