Poland – Construction work for buildings relating to health – zaprojektowanie i wykonanie robót budowlanych w ramach inwestycji pn. „Budowa Centrum kompleksowej opieki nad dziećmi z chorobami reumatycznymi przy Pomorskim Centrum Reumatologicznym im. dr Jadwigi Titz-Kosko w Sopocie Sp. z o.o.” wraz z rozbiórką istniejącego obiektu oraz uzyskaniem pozwolenia na użytkowanie zlokalizowanej w Sopocie przy ul. 23 Marca 93

I.1. The subject of this procedure is the design and execution of construction works and the obtaining of the necessary administrative decisions, agreements and permits, including the permit for use, for the investment entitled "Construction of the Comprehensive Care Centre for Children with Rheumatic Diseases at the Pomeranian Rheumatology Centre named after dr. Jadwiga Titz-Kosko in Sopot Sp. z o.o.", including the demolition of the existing facility, the construction of the Centre building along with the development of the area, technical infrastructure, external installations and connections, the construction of a parking lot, the relocation of the medical oxygen station along with the construction of a canopy with an oxygen decompression chamber and the construction of a drilling platform for the installation of a karst well, to be carried out in Sopot at ul. 23 Marca 93 on plots with cadastral numbers 4/10, 3/5, 3/4. 2. 5. The scope of the works includes: a. demolition of the existing building of the rheumatology and rehabilitation centre for children at the Geriatric Care Centre of the Pomeranian Rheumatology Centre in Sopot, along with the demolition of the development elements, and the development of a multidisciplinary technical design, executive design, workshop designs, other required designs and analyses, and the execution of construction works (including technical and executive designs of installations: water, sewerage, heating, electrical in the scope of switchgears, emergency and evacuation lighting, and SSP for the final arrangement of the second (2) floor), as well as obtaining a valid permit for the use of the building of the Comprehensive Care Centre for Children with Rheumatic Diseases at the Pomeranian Rheumatology Centre named after dr. Jadwiga Titz-Kosko in Sopot Sp. z o.o. along with the technical infrastructure – stage I; b. development of a construction design, multidisciplinary technical design, executive design, workshop designs, other required designs and analyses, obtaining a building permit and all other necessary permits, decisions and agreements, as well as the execution of construction works, and obtaining a valid permit for the use of an above-ground parking lot for 30 parking spaces with a parking system and technical infrastructure, with an underground retention tank for rainwater, a station with an oxygen tank and a canopy with an oxygen decompression chamber – stage II; c. development of the area and greenery, construction of access roads, external installations and connections necessary for the proper functioning of the investment and obtaining a decision on the permit for the use of stages I and II, along with obtaining the necessary permits, decisions, agreements, technical conditions, acceptance and other formal documents required for the execution of construction works covered by stages I and II; d. execution of a drilling platform for the construction of a karst well – implementation within stages I and II; e. development of a construction design, multidisciplinary technical design, executive design, workshop designs, other required designs and analyses, obtaining a building permit and all necessary agreements, decisions, permits, etc., execution of construction works along with obtaining a permit for use within the arrangement of the second (2) floor of the building of the Comprehensive Care Centre for Children with Rheumatic Diseases – stage III; the scope covered by the right of option, as referred to in Section II, point 29 of the SWZ f. provision of maintenance, inspection and conservation services for all systems, installations and equipment, including equipment, the maintenance, inspection and conservation of which are required by regulations and manufacturer's recommendations in order to maintain continuous operability and preserve the warranty conditions, as well as garden maintenance services – during the first three (3) years of the warranty period, within the investment entitled "Construction of the Comprehensive Care Centre for Children with Rheumatic Diseases at the Pomeranian Rheumatology Centre named after dr. Jadwiga Titz-Kosko in Sopot Sp. z o.o." 3. The subject of the contract also includes the exercise of author's supervision by the designers of the Contractor, in accordance with the provisions of the Act of 7 July 1994 – Construction Law, during the execution of construction works carried out on the basis of design documentation prepared by the Contractor within the framework of this contract. II. The Purchaser has provided for possible changes to the contract in the contract template and has determined the type and scope of changes and the conditions for their introduction. III. In the case of a tender submitted by Contractors jointly applying for the award of a public contract, the tender must also include: 1. Declaration JEDZ/ESPD confirming the absence of grounds for exclusion and compliance with the conditions for participation in the procedure, in which each of the Contractors demonstrates the absence of grounds for exclusion and compliance with the conditions for participation in the procedure, to the extent that each of the Contractors demonstrates compliance with the conditions for participation in the procedure – each of the Contractors jointly applying for the award of a public contract submits in their own name. The template for the declaration is Appendix no. 3 to the SWZ. In addition, in order to demonstrate the absence of grounds for exclusion, each of the Contractors jointly applying for the award of a public contract submits in their own name a declaration constituting Appendix no. 3a to the SWZ. 2. Document of authorization of the proxy acting on behalf of the entities applying for joint execution of the contract in the case of submitting a joint tender. The authorization must clearly indicate the actions to be performed, which the proxy is authorized to perform, and the authorization to represent all Contractors jointly applying for the award of a public contract. 3. Declaration, from which it follows which construction works, services or supplies will be carried out by the individual Contractors acting jointly. 4. If the tender of the Contractors jointly applying for the award of a public contract is selected – the Purchaser may, before signing the contract, request a copy of the contract regulating the cooperation of these entities.
