Poland – Refuse and waste related services – Odbiór, transport i zagospodarowanie odpadów komunalnych z terenu Gminy Wiązowna

The subject of the contract is the provision of services related to the collection, transport, and disposal of municipal waste from the area of the Municipality of Wiązowna: 1) Part I: Task 1: Collection, transport, and disposal of municipal waste from properties where residents live, as well as from owners of properties where holiday homes are located or from other properties used for recreational purposes, where municipal waste is generated. Task 2: Collection, transport, and disposal of municipal waste from the area of the Municipality of Wiązowna. 2) Part II: Collection, transport, and disposal of municipal waste from the Selective Waste Collection Point located at ul. Wiązowskiej 2a in Emów (sewage treatment plant area). The detailed description of the subject of the contract is Annex No. 1 to SWZ. 2. The Contractor is obliged to implement the contract on the principles and conditions specified in the Description of the subject of the contract and in the draft provisions of the agreement, which are respectively Annex No. 1 to SWZ and Annex No. 2 to SWZ. 3. A complete offer must include: 1) Offer Form, which is Annex No. 3 to SWZ; 2) Price Form, which is an Annex to the Offer Form. 3) Contractor's Declaration of no grounds for exclusion from the procedure and compliance with the conditions for participation in the procedure for awarding the contract, submitted in the form of the Single European Procurement Document (SEPD) - in accordance with the content of Annex No. 4 to SWZ; 4) Confirmation of the deposit (original deposit document, and in the case of a cash deposit, proof of deposit). 5) In the case of Contractors jointly applying for the award of the contract, a document establishing the Representative to represent them in the procedure for awarding the contract or represent them in the procedure and conclude the contract in the matter of this public procurement; 6) Relevant Power(s) of Attorney - in cases where the authority to sign the offer does not directly result from the document confirming the legal status; 7) Relevant undertaking of the entity on whose technical or professional capabilities or financial or economic situation the Contractor will rely, to make the necessary resources available to it for the purpose of implementing the contract (if applicable) - in accordance with Annex No. 9 to SWZ. 8) In the case of Contractors jointly applying for the award of the contract, a declaration from which it appears what scope of the subject of the contract will be performed by the individual Contractors, based on the template which is Annex No. 10 to SWZ; 9) Declaration regarding the sanctions regulations related to the war in Ukraine regarding the Contractor - Annex No. 11 to SWZ; 10) Declaration regarding the sanctions regulations related to the war in Ukraine regarding the providing entity - Annex No. 12 to SWZ - if applicable.
