Poland – Special-purpose road passenger-transport services – Usługi transportu sanitarnego pacjentów dla Narodowego Instytutu Onkologii im. Marii Skłodowskiej-Curie – Państwowego Instytutu Badawczego Oddziału w Gliwicach

1. The subject of this contract is the provision of patient transport services (with a specialist team and with a basic team) for the National Institute of Oncology named after Maria Skłodowska-Curie - National Research Institute Branch in Gliwice. 2. The description of the subject of the contract and the conditions for the execution of the contract are contained in the annexes: 1) Detailed conditions for the provision of the service – Annex No. 2 2) Product and price specification – Annex No. 3 to SWZ, 3) Draft contractual provisions, which will be incorporated into the contract – Annex No. 5 to SWZ. 3. The contracting authority allows for the submission of equivalent offers - the description of equivalence is contained in Section IV of the SWZ. 4. The contracting authority does not envisage the convening of a meeting of Contractors to explain the content of the SWZ and does not envisage the conduct of a local inspection. 5. The contracting authority does not envisage an electronic auction or supplementary contracts. 6. Information on the joint tendering of Contractors for the award of the contract is contained in Section IX of the SWZ. 7. The information clause resulting from the content of Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, the contracting authority has included in the SWZ. 8. The contracting authority does not require the submission of objections nor does it require the submission of a guarantee of proper contract performance. 9. The contracting authority informs that in this public procurement procedure, the specified activities in the scope of the subject contract will not involve work in the sense of Article 22 §1 of the Labour Code. 10. The period of commitment of the offer is: 90 days. The period of commitment of the offer begins with the expiry of the offer submission deadline. This day is the first day of the period of commitment of the offer. The above means that the period of commitment of the offer expires on 28.02.2027. 11. The contractor is obliged to conclude at their own cost insurance contracts: 1) Mandatory civil liability insurance for the activity carried out in connection with the subject of the contract, in an amount not lower than that resulting from the Regulation of the Minister of Finance of 29 April 2019 on mandatory civil liability insurance of an entity carrying out medical activity (i.e. Journal of Laws of 2025, item 272), if the entity is obliged to conclude such insurance, 2) Voluntary civil liability insurance for the activity carried out and the property owned with a guarantee sum of not less than 300,000 PLN for a single and all events during the insurance period and a deductible not higher than 200 PLN, taking into account damages to the property of patients during medical transport with a minimum limit of 20,000 PLN for a single and all events during the insurance period, 3) Mandatory insurance of motor vehicle owners for those vehicles which the contractor will use in the execution of the subject of the contract. 12.
