4.1 The subject of this procedure is: the supply in 2027 of heating oil to the boilers for the City of Łódź Office and municipal organisational units, supplied by the Contractor to the following locations and in the specified quantities:  supply up to 36.00 m3 (main order up to 26.00 m3, option up to 10.00 m3) of heating oil to the boiler in the building of the City of Łódź Office, ul. Krzemieniecka 2B in Łódź, supplied by the Contractor in batches up to 9.00 m3,  supply up to 29.00 m3 (main order up to 26.00 m3, option up to 3.00 m3) of heating oil to the boiler in the building of Liceum Ogólnokształcące nr XXX im. ks. bpa Ignacego Krasickiego, ul. Sowińskiego 50/56 in Łódź, supplied by the Contractor in batches up to 6.00 m3,  supply up to 8.50 m3 (main order up to 8.0 m3, option up to 0.50 m3) of heating oil to the boiler in the building of Zespół Przedszkoli Miejskich Nr 2, ul. Demokratyczna 61/63 in Łódź, supplied by the Contractor in batches up to 2.30 m3,  supply up to 11.00 m3 (main order up to 9.00 m3, option up to 2.00 m3) of heating oil to the boiler in the building of Przedszkole Miejskie Nr 93, ul. Wichrowa 1A in Łódź, supplied by the Contractor in batches up to 2.00 m3,  supply up to 56.00 m3 (main order up to 39.00 m3, option up to 17.00 m3) of heating oil to the boiler in the building of Specjalny Ośrodek Szkolno - Wychowawczy Nr 6 im. mjr Hieronima Baranowskiego, ul. Dziewanny 24 in Łódź, supplied by the Contractor in batches up to 8.00 m3,  supply up to 25.00 m3 (main order up to 22.00 m3, option up to 3.00 m3) of heating oil to the boiler in the building of Szkoła Podstawowa Nr 120 im. Konstytucji 3-go Maja, ul. Centralna 40 in Łódź, supplied by the Contractor in batches up to 7.00 m3,  supply up to 25.50 m3 (main order up to 22.00 m3, option up to 3.50 m3) of heating oil to the boiler in the building of Szkoła Podstawowa Nr 204 im. Stefana Kardynała Wyszyńskiego, ul. Gajcego 7/11 in Łódź, supplied by the Contractor in batches up to 7.00 m3, 4.2 A detailed description of the subject of the order is in Annex 1 to SWZ and in the contract template Annex 5 to SWZ 6. Gajcego 7/11 Contracts for public procurement concluded as a result of the resolution of the procedure will be signed separately by each of the Contracting Authorities (i.e. each of the Contracting Authorities will independently place a public procurement order) PROCEDURE CONDUCTING AUTHORITY: Public Procurement Department in the Department of Organisation and Citizens' Service of the City of Łódź Office, ul. ks. In accordance with art. 257 of the Pzp Act, the Contracting Authority does not foresee the possibility of invalidating the subject of the procedure if the public funds that the Contracting Authority intended to allocate to finance the whole or part of the order have not been granted to it. 9.3.2 In order to demonstrate the lack of grounds for exclusion from the procedure for awarding the order to the Contractor, the Contracting Authority requires the following documents: 9.3.2.1 A declaration by the contractor in accordance with art. 108 para. 1 point 5 of the Pzp Act, on the lack of affiliation with the same capital group within the meaning of the Act of 16 February 2007 on the protection of competition and consumers (j.t. Dz. U.z 2024 r. poz. 594) with another contractor who submitted a separate offer, or a declaration of affiliation with the same capital group together with documents or information confirming the preparation of the offer independently of another contractor belonging
