Full title: Conducting supervision of the social support network for educators of Children's Homes within the framework of training on medical issues based on EBM for persons participating in the project entitled: "Na ratunek – kadry DIM" co-financed by the European Union, from the European Social Fund Plus within the programme European Funds for Social Development

1. The subject of the present procurement is the conducting of supervision of the social support network for educators of Children's Homes within the framework of training on medical issues based on EBM (Evidence Based Medicine), for persons participating in the project "Na ratunek - kadry DIM", co-financed by the European Union from the European Social Fund Plus within the Programme European Funds for Social Development, planned for implementation.

2. The present procurement is divided into parts. A detailed description of the subject of the procurement in relation to the individual parts of the procurement is contained in Annex 1A-1B and Annex 4 to the SWZ.

3. The contracting authority allows the submission of partial offers for any selected parts of the procurement, with the reservation of the provisions contained in point 5.

4. The contractor is obliged to implement the procurement on the principles and conditions described in the SWZ and in Annex 1A-1B to the SWZ and the contract template, which is Annex 4 to the SWZ.

5. NOTE: 1) due to the organisational assumptions of the training indicated in the detailed schedule of implementation of the training based on EBM, which is an annex to the Description of the subject of the procurement, in particular due to the fact that two groups of participants will work in parallel in the same terms, and meetings with the Supervisor of the support network may take place at the same time, the Contractor ensures the availability of the person designated to implement the given part of the procurement, meeting the minimum substantive requirements specified by the Contracting Authority in Section VIII, point 1, paragraph 4 of the SWZ and in Annex 1B to the SWZ in the scope of the given part of the procurement and the implementation of supervision, for the indicated groups of participants, in the manner and within the deadlines indicated in the detailed schedule of implementation of the training, which is an annex to the Description of the subject of the procurement.

2) having the above in mind, it is not allowed to indicate the same person for the implementation of the procurement – within the parts (supervision) implemented on the same days, i.e. simultaneously for two groups (group IX and for group X) - that is: - in the scope of parts 2, 3 (supervision implemented in the same terms for both groups of participants): if the Contractor submits an offer for the implementation of parts 2 and 3 of the procurement, i.e. within the scope of supervision implemented in parallel for groups IX and X, then for the implementation of part 2, a different person must be designated than in part 3, meeting the minimum substantive requirements specified by the Contracting Authority in Section VIII, point 1, paragraph 4 of the SWZ and in Annex 1B to the SWZ in the scope of the given part of the procurement.

6. Minimum guaranteed level of procurement implementation: The Contracting Authority guarantees the implementation of the procurement at the level of: implementation of 1 hour of supervision for each part separately.
