The subject of the order is the "Regional Health Needs Survey". The subject of the order is to carry out a field survey service. The order concerns the implementation of a study on health and risk factors for persons aged 18 and over, based on a questionnaire prepared by the Contracting Authority. The detailed scope of the subject of the order and the conditions for the execution of the order are described in Section III (Description of the Subject of the Order) and in Section IV (Draft Provisions of the Contract for the Public Order Specification of the Conditions of the Order. The financing source of the order is contract no. FERS.04.15-IP.07-0003/26-00/1094/2026/45 within the framework of the European Social Development Fund 2021 – 2027, co-financed from the European Social Fund Plus.
