The subject of the contract is the supply of dressings, bandages, underlays, medical products and wound care items. 2. The detailed scope of work is described in Annex 1 to the SWZ and to the contract – the assortment-price form, which is an integral part of the contract. All items must be priced in the packages and the subject of the contract must meet the requirements described in Annex 1 – assortment-price form. CPV CODE 33141110-4 Dressings CPV CODE 33141116- Dressing kits 3. The subject of the contract has been divided into 48 parts/tasks. 4. The contracting authority allows partial bids to be submitted. The term partial bid refers to individual parts/packages as set out in Annex 1 to the SWZ. The contractor may submit a bid for any packages or for the entire subject of the contract. 5. All the requirements specified in the documents indicated above constitute minimum requirements, and their fulfillment is mandatory. Failure to meet the above minimum requirements will result in the bid being rejected as non-compliant with the terms of the contract under Article 226(1)(5) of the Pzp Act. 6. The medical devices offered must be approved in the territory of Poland, in accordance with the applicable legal provisions and have documents confirming the completion of the appropriate conformity assessment procedure with the requirements specified in the Act on Medical Devices (Journal of Laws 2024 item 1620) and be marked with the CE mark.
