Full Title: Conducting educational activities as part of the specialist training in the field of psychotherapy for children and youth for persons participating in the project entitled: „Na ratunek – kadry DIM” co-financed by the European Union, from the European Social Fund Plus funds as part of the European Funds for Social Development 2021-2027. 1. The subject of this contract is the preparation and conduct of educational activities on specialist courses as part of the specialist training in the field of psychotherapy for children and youth (Module I), in accordance with the specialist training program in the field of Child and Adolescent Psychotherapy for 2026, approved by the Minister of Health for persons participating in the project entitled: „Na ratunek – kadry DIM” co-financed by the European Union, from the European Social Fund Plus funds as part of the European Funds for Social Development 2021-2027. 2. This contract is divided into parts. A detailed description of the subject of the contract in relation to the individual parts of the contract are Annex 1A-1B and Annex 4 to the SWZ. 3. The contracting authority allows the submission of partial offers for any selected parts of the contract, with the reservation of the entries contained in point 5. 4. The contractor is obliged to implement the contract on the principles and conditions described in the SWZ and in Annex 1A-1B to the SWZ and the contract template, which is Annex 4 to the SWZ. 5. NOTICE: due to the organizational assumptions of the training indicated in the detailed schedule of the implementation of the specialist training, which is an annex to the Description of the subject of the contract, the Contractor ensures the availability of a person designated to implement a given part of the contract, meeting the minimum substantive requirements specified by the Contracting Authority in section VIII point 1 point 4 SWZ and in Annex 1B to the SWZ in the scope of a given part of the contract and the implementation of classes on a specified topic, for the indicated groups of participants, in the manner and within the deadlines indicated in the detailed schedule of the implementation of the specialist training, which is an annex to the Description of the subject of the contract. 6. Minimum guaranteed level of contract implementation: The contracting authority guarantees the implementation of the contract at the level: The contracting authority guarantees the implementation of the contract at the level: implementation of 1 hour of educational activities for Group 2 (in accordance with the detailed schedule of the implementation of the specialist training – annex to the OPZ) in the scope of each part of the contract.
