The subject of the contract is the provision of services consisting of cleaning and maintaining the cleanliness of external hard surfaces and maintaining green areas located within the responsibility of 21. Bazy Lotnictwa Taktycznego in Świdwin, i.e. in the garrison of Świdwin, Mrzeżyno, Trzebiatów and Mirosławiec for the year 2027. 1. Before awarding the contract, the Contracting Authority will invite the Contractor, whose offer was highest rated, to submit within a specified period, not shorter than 10 days, the following current declarations or documents, as of the date of submission, to confirm that the Contractor is not excluded from the proceedings and meets the conditions for participation in the proceedings: a) current information from the National Criminal Register in the scope specified in art. 108(1)(1), (2) and (4) of the Pzp Act, issued no earlier than 6 months before its submission. A Contractor who relies on the resources of other entities, in order to demonstrate the absence of grounds for exclusion in respect of them, submits the above information from the National Criminal Register also in respect of those entities. b) A declaration by the Contractor/entity providing resources on the currency of the information contained in the declaration referred to in art. 125(1)/125(5) of the Act, in the scope of the grounds for exclusion from the proceedings indicated by the Contracting Authority: - art. 108(1)(3) of the Pzp Act: - art. 108(1)(4) of the Act regarding the decision prohibiting the pursuit of a public contract as a preventive measure, - art. 108(1)(5) of the Act regarding the conclusion with other contractors of an agreement aimed at disrupting competition, - art. 108(1)(6) of the Act, and - art. 7(1) of the Act of 13 April 2022 on special solutions in the field of counteracting support for aggression against Ukraine and serving to protect national security (Journal of Laws 2022, item 835), (hereinafter: "Act"), - art. 5k of Council Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine (hereinafter: "Regulation"), consistent in content with Annex No. 11 to SWZ c) declarations by the Contractor (entity providing resources), in the scope of art. 108(1)(5) of the Act, of not belonging to the same capital group within the meaning of the Act of 16 February 2007 on the protection of competition and consumers (Journal of Laws 2023, item 1689), with another contractor who submitted a separate offer, partial offer or application for admission to the proceedings, or a declaration of belonging to the same capital group together with documents or information confirming the preparation of the offer, partial offer or application for admission to the proceedings independently of another contractor belonging to the same capital group, consistent in content with Annex No. 12 to SWZ, d) a list of services performed, and in the case of periodic or continuous services also performed, in the last 3 years, and if the period of conducting business is shorter – in that period, together with the indication of their value, subject, date of performance and entities, for which the services were performed (consistent in content with Annex No. 6 to SWZ) and attaching evidence specifying whether the services were performed or are being performed properly, where the evidence referred to are references or other documents issued by the entity, for which the services were or are being performed, and in the case of periodic or continuous services being performed, references or other documents confirming their proper performance should be issued no earlier than 3 months before the deadline for submitting offers or applications for admission to the proceedings). e) a list of technical equipment available to the Contractor for the performance of the public contract together with information on the basis for disposing of these resources, consistent in content with Annex No. 7 to SWZ. No later than on the date of signing the contract, the Contractor is obliged to present: a) a consortium agreement - in the case of a consortium, a civil-law agreement - in the case of a civil partnership. The submitted documents must clearly indicate the scope of cooperation between the contractors jointly applying for the award of the contract, b) a declaration of employment on the basis of an employment contract of persons performing activities for the Contracting Authority together with a list of employees employed under an employment contract (only first name and surname).
