The subject of the contract is: the supply of single-use medical equipment. 2. The contract has been divided into 9 lots. Lot 1 – Hygienic aprons. Lot 2 – Surgical clothing. Lot 3 – Single-use surgical caps - male. Lot 4 – Single-use protective sheet. Lot 5 – Transferable patient underlay. Lot 6 – Patient thermal blanket. Lot 7 – Non-woven sheet. Lot 8 – Finger splint. Lot 9 – Single-use aprons. 1. The contractor is obliged to submit on the day of submission of offers: a) Offer form using the template constituting Annex 1 to SWZ. b) Price form using the template constituting Annex 1A to SWZ. c) Contractor's declaration that the conditions of participation in the procedure are met and is not subject to exclusion (JEDZ) - according to the template constituting Annex 2 to SWZ. d) Declaration of demonstrating the absence of grounds for exclusion – the content of the declarations is included in the offer form. e) Subject evidence, as mentioned in Section VIII.1.1) SWZ - the content of the declarations is included in the offer form. f) Subject evidence, as mentioned in Section VIII.1), 2), 3) SWZ. g) Power of attorney (if applicable) NOTICE!!! Annexes 1 and 1A to SWZ constitute the content of the offer and therefore are not subject to supplementation. The absence of these documents constitutes the invalidity of the submitted offer. Subject evidence, as mentioned in Section VIII.1.1) SWZ (declaration), constitute an integral part of the offer and therefore are not subject to supplementation. LIST OF SUBJECT EVIDENCE 1. The ordering party, in order to confirm the conformity of the offered supplies, requires the submission together with the offer of the following subject evidence: 1) Declaration that the offered assortment has been approved and introduced into circulation in accordance with the provisions of the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974) and has current declarations of conformity and certificates of the notified body for medical devices) - the content of the declaration is included in the offer form, constituting Annex 1 to SWZ. 2) Producer's catalogue cards/other producer's materials confirming the parameters required in the description of the subject of the contract, containing the catalogue numbers of the offered assortment and photographs/drawings of the offered assortment - applies to all lots. *Please assign documents to a given position in the lot. 3) Sample samples of 1 pc. of the offered assortment - applies to lots 1, 2, 3, 4, 5, 6, 7. 2. Subject evidence prepared in a foreign language is submitted together with a translation into Polish. 3. The ordering party requires that the subject evidence be prepared in electronic form and signed with a qualified signature by the contractor. 4. In the event that the subject evidence has been prepared in electronic form and signed with a qualified signature by the issuer (authorized entity), it should be submitted in such a form. 5. In the case where the subject evidence has been issued in paper form, a digital representation of the document signed with a qualified signature by the contractor should be submitted. 6. The ordering party requires that the subject evidence confirm the actual status as of the date of submission of offers. 7. The ordering party requires that the subject evidence be up-to-date as of the date of submission/ supplementation. 8. In the event that the contractor does not submit the subject evidence together with the offer or the submitted subject evidence is incomplete, the ordering party will call for their submission or supplementation within the specified deadline - applies to documents mentioned in point 1, sub-point 2, 3. Offers should be submitted via the eZamawiajacy Platform available at https://spskgruca.ezamawiajacy.pl/servlet/HomeServlet. 2. Samples should be delivered to the hospital's office or the Public Procurement Department (from 7:30 am to 3:05 pm).
