Poland – Miscellaneous computer equipment – Zakup wraz z dostawą sprzętu komputerowego w ramach zadania pn.: Doposażenie „Pracowni dydaktycznych (tj.:..) dla potrzeb 3 techników wchodzących w skład CKZiU ul. Kilińskiego 25 w Sosnowcu w ramach realizowanego projektu „Zdobądź zawód i kwalifikacje - transformacja CKZiU ul. Kilińskiego 25”.

EVIDENCE AND DOCUMENTS TO BE SUBMITTED WITH THE TENDER 1.1. The tender must be submitted electronically (i.e. with a qualified electronic signature). The tender must be prepared in accordance with the template constituting Annex 1 to the SWZ. 1.2. The contractor must attach to the tender a declaration of non-exclusion in the scope indicated in Section III, paragraph 2 of the SWZ: a) point 2.2. SWZ [in the scope of the grounds for exclusion from art. 108(1) points 1) – 6) of the Pzp Act] – the declaration must be submitted in the form of the Single European Standard Form [hereinafter: “SES”] – Annex 3 to the SWZ; b) point 2.3. SWZ [in the scope of the grounds for exclusion from art. 7(1) of the Act of 13 April 2022 on special solutions in the field of counteracting support for aggression on Ukraine and serving to protect national security] – the declaration must be submitted in the form of the SES, indicating the appropriate answer in Part III, Section D – Annex 3 to the SWZ; c) point 2.4. SWZ [in the scope of the grounds for exclusion from art. 5k) of Council Regulation (EU) No 833/2014 of 31 July 2014 as amended by Regulation 2025/2033 (OJ L, 2025/2033, 23.10.2025)] – the declaration must be submitted in accordance with the template constituting Annex 4 to the SWZ. These declarations constitute evidence confirming the absence of grounds for exclusion, and in the case of submitting a certificate – a declaration in the scope in which the certificate does not prove compliance with these requirements - - Part II, Section A, SES (line regarding information whether the contractor is entered in the official register of approved contractors or has an equivalent certificate). 1.3. The declaration referred to in Section IV, paragraph 1, point 1.2. SWZ lit. a) – b) must be submitted in the form of the European Single Procurement Document, prepared in accordance with the template of the standard form specified in the Implementing Regulation of the European Commission issued pursuant to art. 59(2) of Directive 2014/24/EU, hereinafter referred to as the “Single Document”. The contractor is obliged to submit the SES in the scope indicated in Annex 3 to the SWZ. The Contracting Authority informs that the Instructions for completing the SES are available on the Public Procurement Office website. 1.4. The SES template provided by the Contracting Authority is in accordance with the Implementing Regulation of the European Commission (EU) 2016/7 of 5 January 2016 establishing the standard form of the European Single Procurement Document. The contractor is obliged to complete the SES to such an extent that the Contracting Authority is able to verify whether the contractor is not subject to exclusion in the scope described in this SWZ. 1.5. Declarations referred to in Section IV, paragraph 1, point 1.2. SWZ are submitted separately: 1) Contractor/each of the Contractors jointly applying for the award of the contract. In such a case, the declaration confirms the absence of grounds for exclusion of the Contractor in the procedure. 1.6. The contractor is obliged to attach to the tender: 1) declarations referred to in Section IV, paragraph 1: point 1.2. a) – b) SWZ – in the form of the SES (according to Annex 3 to the SWZ), and point 1.2. c) SWZ – according to the template attached to the SWZ; 2) an extract or information from the National Court Register, Central Information and Business Activity Register or another relevant register confirming that the person acting on behalf of the contractor is authorised to represent him; 3) a power of attorney or another document confirming the authorisation to represent the Contractor, if the authorisation of the person submitting the tender does not result from the documents described in point 2); 4) in the case of contractors jointly applying for the award of the contract: a) a power of attorney, from which it will result the authorisation to represent them in the procedure for the award of the contract or to represent in the procedure and conclude a contract for the public procurement. The power of attorney should include in particular the indication: - of the public procurement procedure to which it relates, - of all contractors jointly applying for the award of the contract listed by name with the address of the registered office, - of the appointed representative and the scope of his authorisation; b) a declaration, referred to in Section I, paragraph 4, point 4.4. SWZ (the Contracting Authority allows the submission of this declaration on the tender form). 1.7. The contractor is not obliged to submit the documents referred to in paragraph 1.6. point 2), if the Contracting Authority can obtain them through free and publicly available databases, provided that the contractor indicates the data enabling access to these documents. 1.8. The provisions of paragraph 1.6. point 3) shall apply accordingly to the person acting on behalf of the contractors jointly applying for the award of the contract.
