1. The subject of the procedure is implemented as part of the targeted grant from the Minister of Health for the task entitled „purchase of equipment and apparatus” within the National Transplant Programme by purchasing equipment and apparatus for lung transplant centres with the highest transplant activity. 2. The subject of the contract is the delivery of medical equipment with the technical parameters specified in Annex No. 4a, 4b, 4c, 4d, 4e and 4f to SWZ, hereinafter referred to as „equipment”, along with installation and training of the Contracting Authority's staff. 3. The subject of this contract includes the following parts: Part 1 – Transesophageal Probe (1 unit) with the technical parameters specified in Annex No. 4a to SWZ; Part 2 – Ventilator (1 unit) with the technical parameters specified in Annex No. 4b to SWZ; Part 3 – Bedside Rotor (1 unit) with the technical parameters specified in Annex No. 4c to SWZ; Part 4 – Videobronchoscope (1 unit) with the technical parameters specified in Annex No. 4d to SWZ; Part 5 – Patient Lift (1 unit) with the technical parameters specified in Annex No. 4e to SWZ; Part 6 – Optical Bronchoscope (1 unit) with the technical parameters specified in Annex No. 4f to SWZ; 4. The above-mentioned equipment must meet the boundary conditions specified in Annex No. 4a, 4b, 4c, 4d, 4e and 4f to SWZ. Failure to meet even one of the boundary conditions specified in Annex No. 4a, 4b, 4c, 4d, 4e and 4f to SWZ will result in the rejection of the offer.
