The subject of the contract is the delivery of medical equipment and consumable materials - accessories in 5 Parts: Part 1 – Video-cystoscope with control unit - 1 set with accessories - (Part 1 Item 2); Part 2 – Holmium laser - 1 unit with accessories - (Part 2 Item 2); Part 3 – Software for MRI/USG image fusion for transrectal prostate biopsy with equipment – 1 set; Part 4 – Semiconductor laser – 1 unit with accessories - (Part 4 Item 2); Part 5 - C-arm X-ray machine with surgical table - 1 set. 2. The detailed description of the subject of the contract are Annexes 2.1. - 2.5. to SWZ – Technical Parameter Forms (concerning Parts 1 – 5 excluding accessories) and Annex 2 to SWZ – Assortment and Price Form (concerning Part 1 Item 2, Part 2 Item 2 and Part 4 Item 2 – "accessories - consumable materials") – respectively within the scope of the Part to which it relates.
