Description

The subject of the contract is the supply of sewing materials, staplers, and loads used for surgical procedures, sets for treating pelvic floor dysfunction, and various medical products (CPV: 33.10.00.00-1, 33.14.11.11-1, 33.14.11.21-4, 33.14.11.27-6, 33.15.82.10-7) for the needs of Copernicus Podmiot Leczniczy Sp. z o.o. in Gdańsk, the detailed description, assortment and estimated quantities of which are contained in the Price and Assortment Form - Annex 1 to the SWZ (parts 1-50). The Contractor, by filling in the positions in the column "Indicate yes/no", confirms compliance with the requirements specified by the Contracting Authority – if applicable. The offered contract items must be complete, of high standard in terms of both quality and functionality, and free from material, design and legal defects. The offered contract item must be approved for trade and use in accordance with the applicable law. The contract item must be in accordance with the Act of 7 April 2022 on medical devices (Journal of Laws 2022, item 974 of 2022.05.09). In all records of the SWZ and its annexes, where the Contracting Authority refers to standards, approvals, technical specifications or reference systems or indicates trademarks, parameters or sources of origin (names of manufacturers or devices), in accordance with art. 99 para. 5 and art. 101 para. 4 of the Pzp Act, the Contracting Authority allows equivalent solutions. The Contracting Authority does not reserve the possibility of tendering exclusively by Contractors referred to in art. 94 of the Pzp Act. The Contracting Authority does not set requirements referred to in art. 96 of the Pzp Act. In order to submit an offer, the Contracting Authority does not provide for the Contractor to carry out a local inspection or for the Contractor to check the documents necessary for the implementation of the contract available at the Contracting Authority's premises. In order to confirm that the offered supplies meet the requirements specified by the Contracting Authority, the Contracting Authority requires the submission of subject-specific evidence together with the offer: technical cards of the manufacturer of the product or catalogue cards of the manufacturer of the product for the offered part and position. Each side of the cards should be described which range and position it relates to, samples of the offered assortment (described in a way that allows identification): - relates to: part no. 1 pos. 1 - 2 sachets, part no. 2 pos. 14 - 2 sachets, pos. 20 – 2 sachets, part no. 3 pos. 1 – 2 sachets, part no. 4 pos. 9 – 2 sachets, pos. 11 – 2 sachets, part no. 7 pos. 10 - 2 sachets, pos. 13 – 2 sachets, part no. 10 pos. 30 – 2 sachets, pos. 31 – 2 sachets, pos. 34 – 2 sachets, - part no. 11 pos. 1 - 2 sachets, part no. 12 pos. 2 – 2 sachets, pos. 3 – 2 sachets, pos. 4 – 2 sachets, part no. 19 pos. 2 – 3 sachets. The samples provided must be in accordance with the description contained in Annex 1 – Price and Assortment Form. Submission of samples, in accordance with the provisions of § XII SWZ. The Contracting Authority, with regard to the requirement to submit samples, waives the requirement to use electronic communication means. In the case of offering equivalent solutions – other than those specified in the SWZ – the offer should include documents confirming that the equivalent solutions applied meet the requirements of the Contracting Authority (e.g. descriptions, catalogue cards, technical cards). Before selecting the most advantageous offer, the Contracting Authority will call the Contractor, whose offer has been highest rated, to submit, within a period not shorter than 10 days, current subject-specific evidence specified in § X SWZ. The Contracting Authority does not require security for the proper performance of the contract. The method of implementing the contract item is specified in the contract projects contained in Annex 5 to the SWZ.
