Poland – Orthopaedic implants – Dostawa implantów dla Dolnośląskiego Centrum Rehabilitacji i Ortopedii Sp. z o.o. w Kamiennej Górze

1. The subject of this contract is the supply of implants for Dolnośląskie Centrum Rehabilitacji i Ortopedii Sp. z o.o. at J. Korczaka 1, Kamienna Góra. The subject of the contract is divided into twelve (12) parts: Part 1 Hip joint endoprosthesis, Part 2 Revision hip joint endoprosthesis, Part 3 Knee joint endoprosthesis, Part 4 Unicompartmental knee joint endoprosthesis, Part 5 Acetabular cup, Part 6 Shoulder joint endoprosthesis, Part 7 Elbow joint endoprosthesis, Part 8 Hypoallergenic elbow joint endoprosthesis, Part 9 Head of the radius bone prosthesis, Part 10 Ankle joint prosthesis - anterior approach, Part 11 Wrist endoprosthesis, Part 12 Bone cement. 2. The contracting authority reserves the right to exercise an option. 3. The scope of the basic and optional contract and the circumstances for exercising the option are specified in Part II SWZ (Subject of the contract) and in Part IV SWZ (Draft contract). 4. The offered products specified in Annexes 1.1 – 1.12 to the Tender Form must meet the legally binding requirements for placing on the market in accordance with the Act of 07.04.2022 on medical devices (i.e. Journal of Laws of 2024, item 1620, as amended) and must have the required documents confirming this fact. 5. The contractor will be obliged to deliver the specified quantity of items, packages, sets, etc. of a given type of product, regardless of the method of their packaging. The contractor will be obliged to unload and bring in the goods. 6. The contractor is obliged to offer the products listed in Annexes 1.1 – 1.12 to the Tender Form in packages ensuring their full use within their shelf life. 7. A detailed description of the subject of the contract is contained in Part II SWZ (Subject of the contract) and in Annexes 1.1 - 1.12 to the Tender Form.
