Poland – Medical consumables – Dostawa wyrobów medycznych w tym płynów do perfuzji narządowej, środków do pielęgnacji pacjentów, bezwęzłowych urządzeń do zamykania ran, akcesoriów okulistycznych i stosowanych w kardiochirurgii, infuzorów oraz soczewek typu EDOF na potrzeby Klinik USK-2 PUM w Szczecinie.

1. The subject of the contract is the supply of medical products, including organ perfusion fluids, patient care products, sutureless wound closure devices, ophthalmic accessories and those used in cardiothoracic surgery, as well as infusion pumps and EDOF lenses for the needs of the USK-2 Clinics PUM in Szczecin. 2. Whenever the term "products" is used in the rest of the SWZ, it should be understood to mean the products indicated in Annex No. 2 (FCJ) to the SWZ; 3. The description of the subject of the contract, detailed requirements and estimated quantities relating to the products are included in the unit price form (Annex 2 to the SWZ, hereinafter also referred to as FCJ). This form must be submitted together with the tender form (Annex 1 to the SWZ, hereinafter also referred to as "FO") 4. The Purchaser requires the offer of: a. medical products within the meaning of the Act on Medical Devices (Dz.U. 2024.1620 as amended), hereinafter: UoWM), introduced to the market and use in the territory of RP in accordance with the applicable regulations - in the scope of all tasks, all positions with the exception of point 4. b. cosmetic – task no. 4 position 1 5. The Purchaser requires that the period of usability of all offered products is at least 12 months, counted from the date of delivery to the Purchaser's headquarters. 6. The quantities of products given in the unit price form are indicative and the Purchaser has the right to order smaller quantities, provided that they are not less than 60% of the value of the contract/task. 7. The Purchaser, pursuant to art. 30 para. 2 of the PZP Act, awards the contract in parts, each of which is the subject of a separate procedure. 8. The contract consists of 10 tasks, the detailed description of which is contained in the FCJ 9. The Purchaser does not allow the submission of variant offers.
