The subject of the contract is: the purchase and successive delivery of single-use medical equipment. 2. A detailed description of the subject of the contract in terms of the required parameters is specified in Annexes No. 2.1 – 2.3 to SWZ – Description of the Subject of the Contract (contract/ package/task No. 1 – 3) – also referred to as “Annex No. 2 to SWZ”. 3. Place of execution of the contract: ul. Grenadierów 51/59, 04-073 Warsaw. 4. Successive deliveries of the subject of the contract are to be made to the Hospital Pharmacy located in Pavilion III of the Hospital on the ground floor and to the warehouse located in the basement of the Hospital, with the Contracting Authority indicating each time if the delivery place will be the warehouse. 5. Acceptance of the subject of the contract will take place in the premises accessible to the Contractor's (suppliers, couriers) staff. 6. The detailed conditions for the performance of the service are specified in the contract project constituting Annex No. 4 to SWZ. 7. The Contracting Authority allows for the possibility of submitting partial offers (7 – tasks / parts of the contract), in accordance with the division specified in Annex No. 2 to SWZ constituting the Description of the Subject of the Contract form: 1) Task No. 1 – Tracheotomy Set; 2) Task No. 2 – Ports, catheters, needleless systems; 3) Task No. 3 – Sterile ultrasound probe cover sets.
