Subject and scope of the order: The subject of the order is to conduct a nationwide, quantitative, territorial survey on the health and risk factors among adult residents of Poland, using a questionnaire prepared by the Contracting Authority. The thematic scope of the survey particularly covers: general health status, self-assessment, the occurrence of chronic diseases and painful conditions, mental condition and sleep quality, accidents and injuries, lifestyle and risk factors: the use of traditional tobacco products and their substitutes (including e-cigarettes/heaters), physical activity, alcohol and psychoactive substance consumption, anthropometric measurements (BMI, weight, height) and dietary habits, environmental conditions and harmful factors at the workplace, use of healthcare services (public within the framework of the NFZ and private), access to specialists and POZ, barriers, delays and treatment refusals, health prevention, vaccinations and attitudes towards vaccinations, well-being, social support, sense of exclusion and economic-demographic situation. The research tool is a questionnaire consisting mainly of closed questions (single/multiple choice) and optionally 1–2 open questions. The estimated average duration of one interview is approximately 35 minutes.

2. Methodology and sample selection

Research technique: computer-assisted personal interviews (CAPI – Computer-Assisted Personal Interviewing).

Population under study: residents of Poland aged 18 and over, living in individual households, speaking Polish.

Sample size: N = 3,000 respondents.

Sample structure: a representative nationwide quota sample divided by gender (1,500 women, 1,500 men) and age groups: 18–39 years (800 individuals), 40–59 years (800 individuals), 60–74 years (800 individuals), 75–84 years (400 individuals) and 85+ years (200 individuals).

Operator and sampling scheme: GUS address database; sampling of starting points representative for voivodeships and size classes of localities (agglomeration village, non-agglomeration village, city up to 50,000, 50,000–200,000, 200,000+); implementation of interviews using the random route method (bundles of 5–7 interviews).

Representativeness: The contractor is obliged to ensure the representativeness of the sample in terms of age, gender, education and place of residence (voivodeship and size class of locality).

3. Main obligations of the Contractor and final products

Preparatory stage: submitting any comments on the questionnaire (1 working day), submitting a detailed schedule of the study (within 3 working days), programming an electronic test version of the CAPI questionnaire (within 5 working days) and conducting training for interviewers with the participation of the Contracting Authority.

Field implementation stage: conducting CAPI interviews along with submitting progress reports every 10 working days (demo-geo structure, interview duration statistics).

Final products: a report summarising the full study, a tabular presentation of the results, a data set in CSV and SPSS formats with the assigned TERC identifier (TERYT register) for each respondent.

4. Order execution deadline

The deadline indicated in the Contractor's offer – no longer than 55 working days from the date of conclusion of the contract and no later than 15 December 2026 (the deadline includes all implementation phases, including verification and acceptance procedures for reports).
