Poland – Building construction work – Opracowanie dokumentacji projektowej (wraz z pełnieniem nadzoru autorskiego na etapie realizacji zadania) oraz wykonanie robót budowlanych, w formule „zaprojektuj i wybuduj” w ramach projektu pn.: Budowa w Zielonej Górze przy ul. Wyspiańskiego nowego budynku na potrzeby Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Centrum Leczenia Dzieci i Młodzieży w Zaborze”.

The subject of the contract is the preparation of project documentation (including author's supervision during the task implementation phase) and the execution of construction works, in the “design and build” formula, as part of the project entitled: "Construction of a new building in Zielona Góra, at Wyspiańskiego Street, for the needs of the Independent Public Health Care Facility Centre for the Treatment of Children and Youth in Zaborze". The intention of the Independent Public Health Care Facility Centre for the Treatment of Children and Youth (hereinafter referred to as the Contracting Authority) is to build a new headquarters for the Hospital located in Zielona Góra. As part of the new facility, a comprehensive psychiatric treatment centre for children and youth will be created, along with educational, administrative, day care, and permanent stay wards. The scope of the works should be adapted to the requirements of the Contracting Authority presented in the Functional-User Programme. The Programme specifies the requirements and expectations of the Contracting Authority for the subject investment. The Programme precisely defines the scope, quality, and requirements of the equipment to be supplied and installed as part of the implementation of the Investment. The Contractor, as part of the project implementation, should detail and develop the functional layout specified in the Programme, in a manner consistent with the aforementioned regulations and conditions specified for the individual devices to be installed. The actions of the Contractor and the results of his work must be in accordance with the applicable legal order. The Functional-User Programme serves to estimate the planned costs of design and construction works and to prepare an offer, especially in terms of calculating the offer price. As part of the planned undertaking, the following activities should be performed: 1. Pre-design works, including, among others: - preparation of a map for design purposes, - preparation of documentation of soil investigation, geotechnical opinion and geotechnical project - preparation and obtaining a decision to approve the geological works project to determine the geological-engineering conditions - submission of geological works - execution of geological works - preparation and obtaining a decision to approve the geological-engineering documentation - and other necessary investigations or measurements for the implementation of the planned undertaking; - preparation of a dendrological report along with obtaining the necessary opinions regarding tree cutting - submission of a water law application for dewatering of excavations (if required) 2. Preparation of the building design along with the necessary agreements in the form of: - land development project, - architectural and building design, - attachments to the building design, - technical design 3. Obtaining a building permit decision – under the responsibility of the Contractor; 4. Submission of the intention to carry out construction works not requiring a building permit, such as networks and other necessary for the implementation of the planned undertaking 5. Preparation of the executive projects in the scope necessary for the implementation of the investment; 6. Obtaining a decision allowing the cutting of trees and shrubs (administrative fees and resulting from the decision allowing the cutting of trees lie with the Contracting Authority) 7. Preparation of the compensatory planting project along with obtaining the necessary opinions and agreements (assuming the possibility of carrying out plantings on the land parcels with inventory numbers 50/10, 50/11, 50/12, 50/13, 50/20, 50/22, 50/23, 50/24, cadastral area 16, Zielona Góra, (the execution of compensatory plantings lies with the Contracting Authority) 8. Execution of the entire investment in accordance with the building permit and the prepared designs; 9. Delivery and installation of permanently mounted equipment; 10. Obtaining all the necessary agreements, acceptances for the use of the facility and obtaining a building permit.
