Poland – Medical consumables – Dostawa materiałów medycznych wykorzystywanych w chirurgii ginekologicznej oraz histeroskopy jednorazowe z narzędziami i sterownik video do obsługi histeroskopu

1. The subject of the procurement is the supply of medical materials used in gynaecological surgery and single-use hysteroscopes with instruments and a video controller for operating the hysteroscope. 2. The detailed description of the subject of the procurement includes a price-list form that is an attachment to the SWZ. 3. The subject of the procurement will be executed successively from the date of the contract conclusion for a period of 12 months. 4. The contracting authority does not allow the submission of partial offers within the range of the price-list forms. 5. The contracting authority does not allow the possibility of submitting alternative offers. 6. Each Contractor has the right to submit only one offer. The submission of more than one offer by one Contractor or an offer containing alternatives will result in the rejection of all offers submitted by that Contractor. 7. The range offered in the tender procedure (listed in the Price Lists) must be consistent with the description of the subject of the procurement and must meet the conditions for admission to trade in healthcare facilities in accordance with the applicable legal provisions. 8. Failure to meet any of the required parameters gives the contracting authority the basis to reject the offer. The contracting authority allows the offer of equivalent products in relation to the positions described in the Price Lists using trade names or designations indicating a specific product (e.g. in Task 1, 3 and 4), provided that the technical, quality and functional parameters are not worse than those indicated in the description of the subject of the procurement, in accordance with art. 99, para. 5–6 of the Pzp. The burden of proof of equivalence lies with the Contractor submitting the offer.
