The subject of the present tender procedure is: Purchase of a Medical Assistant integrated with the HIS system from Clininet, hereinafter referred to as the "Software" in accordance with the type of assortment and quantities specified in the "Price and Quantity Form" attached as Annex 2 to the SWZ and in accordance with the "Offer Form", attached as Annex 1 to the SWZ as well as the Subject of the Order Description, attached as Annex 1.a and Annex 1.b – Warranty and Service Conditions for the Independent Public Health Care Facility of the University Clinical Hospital No. 2 of the Medical University of Lodz, ul. Żeromskiego 113, 90-549 Łódź, Independent Public Health Care Facility of the University Clinical Hospital No. 1 named after Norbert Barlicki of the Medical University of Lodz, ul. Kopcińskiego 22, 90-153 Łódź and Independent Public Health Care Facility of the Central Clinical Hospital of the Medical University of Lodz ul. Pomorska 251, 92-213 Łódź. In the Price and Quantity Form, in the appropriate columns, the quantities that will be ordered by the Independent Public Health Care Facility of the University Clinical Hospital No. 2 of the Medical University of Lodz, Independent Public Health Care Facility of the University Clinical Hospital No. 1 named after Norbert Barlicki of the Medical University of Lodz and Independent Public Health Care Facility of the Central Clinical Hospital of the Medical University of Lodz are indicated. Each of the Orderers signs a separate contract regarding the order in the scope of quantities specified in the Price and Quantity Form for the given hospital. The implementation of the subject of the procedure is co-financed from the funds of the Medical Research Agency within the competition no. ABM/2023/2 Creation and Development of Regional Digital Medicine Centers. Project name: Medical University of Lodz – Digital Medicine Center (MULDiMediC).
