The subject of the Contract is the delivery, installation and commissioning of 1 unit of a Premium Class USG device along with training. As part of this Contract, the equipment, delivery of the equipment to the Buyer, installation, commissioning of the equipment, connection of the equipment to the existing RIS/PACS system in the Hospital and training of the personnel must be included. 2) The Buyer does not allow partial bids to be submitted. The Contract has not been divided into parts. The Contractor can submit only one bid. 3) The detailed description of the Contract is contained in Annex 4, which is an integral part of the SWZ. This description should be read together with any changes to the content of the SWZ, which may result from answers given to questions from Contractors. 4) The offered equipment must be a medical device approved for sale and use in Poland in accordance with the provisions of the Act of 07.04.2022 on medical devices and with the Regulation of the European Parliament and of the Council (EU) 2017/745 of 5.04.2017 on medical devices. 5) The offered equipment must be factory new (year of manufacture: 2026, unused, complete and no additional elements and accessories need to be purchased for its commissioning and use as intended. The device or any of its components, equipment, etc. must not be refurbished, second-hand or previously used by another user. 6) The required payment term is up to 30 days.
