Poland – Special-purpose road passenger-transport services – Świadczenie usług w zakresie transportu sanitarnego (POWTÓRZONY)

The subject of the order is the provision of services in the field of sanitary transport, in accordance with the requirements specified in Annex No. 3 to the SWZ – Contract Project. 2. The Purchaser does not allow partial offers to be submitted. Offers not containing the full scope of the subject of the order will be rejected. 3. In the procedure for awarding a contract with a value equal to or exceeding the EU thresholds, the offer and the declaration referred to in Article 125(1) shall be submitted, under pain of invalidity, in electronic form (Article 63(1) Pzp). 2. The method of preparing electronic documents, declarations or electronic copies of documents or declarations must be in accordance with the requirements specified in the Regulation of the Minister of Development, Work and Technology of 23 December 2020 on the subject of personal evidence and other documents or declarations which the Purchaser may require from the Contractor (Dz.U. poz. 2415 ze zm.) and the Regulation of the President of the Council of Ministers of 30 December 2020 on the manner of preparing and transmitting information and technical requirements for electronic documents and electronic communication means in the procedure for awarding a public contract or tender (Dz.U. poz. 2452). 3. The electronic communication medium, serving to submit an offer by the Contractor, is its correct submission on the Platform available at https://szpitalswidnica.logintrade.net/rejestracja/ustawowe. in the row marked with the title and the case number in accordance with this html procedure (https://szpital-swidnica.logintrade.net/zapytania_email,236510,7d9a76ef3ecf454364dceb7947a4d815.html). 4. The Purchaser will transmit information to contractors in electronic form via the Tendering Platform. Information regarding responses to questions, changes in specifications, changes in the deadline for submission and opening of offers, the Purchaser will post on the Tendering Platform on the card of the conducted procedure. Correspondence, which, in accordance with the applicable provisions, is addressed to a specific Contractor, will be sent in electronic form via the Platform to the specific Contractor. 5. Procurement procedures are conducted using electronic communication means. Communication between the Purchaser and the Contractors takes place using the SP ZOZ Procurement Platform in Świdnica at https://szpitalswidnica.logintrade.net/ The connection uses the encrypted HTTPS protocol. Data encryption is carried out using the SSL protocol. The SSL certificate ensures the confidentiality of data transmitted over the Internet. A Contractor having an account on the SP ZOZ Tendering Platform in Świdnica: has access to the possibility of submitting, withdrawing an offer, as well as functionalities allowing to ask questions about the content of the SWZ and communicate with the Purchaser in other areas. The Purchaser allows communication with contractors to the email address given in the SWZ (with the reservation that the offer together with the attachments must be submitted on the platform). 6. After creating or generating an electronic document by the Contractor, the Contractor signs this document with a qualified electronic signature, issued by a qualified trust service provider, being a subject providing certification services – electronic signature, meeting the security requirements specified in the Act of 5 September 2016 on trust services and electronic identification (DZ.U. z 2024 poz. 1725 t.j. ze zm). 7. The exercise by a data subject of the right to rectification or supplementation, referred to in Article 16 of Regulation 2016/679, may not result in a change in the result of the award procedure or a change in the provisions of the contract for a public contract in a scope inconsistent with the law. 2. In the procedure for awarding a contract, the submission of a request to restrict processing, referred to in Article 18(1) of Regulation 2016/679, does not restrict the processing of personal data until the completion of that procedure.
