The subject of the contract has been divided into 2 Parts: 1.1. Part I: Update of the PZO documentation preparation along with the performance of nature expertise for the Natura 2000 area Ostoja Popradzka PLH120019 in the field of chiropterology; 1.2. Part II: Update of the PZO documentation preparation along with the performance of nature expertise for the Natura 2000 area Ostoja Popradzka PLH120019 in the field of ichthyology. 1.3. The Contracting Authority, pursuant to art. 95(1) of the Act, requires that the Contractor or subcontractor employs, on the basis of an employment relationship, persons designated by the Contractor or subcontractor to perform tasks such as: collecting literature data, desk work, completing the PZO documentation template, if it involves performing work in the manner specified in art. 22(1) of the Act of 26 June 1974, the Labour Code (i.e. Journal of Laws of 2025, item 277, 807, 1423, 1661, of 2026, item 25). The above does not apply to situations where the Contractor independently implements the contract or through a person employed on the basis of a civil law contract (e.g. a person independently organizing their time and scope of expert work as ad hoc activities, the result of which is to achieve a specific result) other than an employment relationship. A detailed description of the contract is in Annex No. 9 to the SWZ. II. The tender in the procedure is submitted, under pain of invalidity, in electronic form, i.e. in electronic form with a qualified electronic signature. III. 1. Documents submitted by the Contractor together with the tender: 1) completed Tender Form (Annex No. 1 to the SWZ). If the Contractor wishes to obtain additional points in the second tender evaluation criterion - completed list of additional expert experience (Annex No. 1A to the SWZ); 2) power of attorney or other document confirming the authorization to represent the Contractor – if the tender or statements or documents attached thereto are signed by a representative. 3) power of attorney to represent all Contractors jointly applying for the award of the contract, or a cooperation agreement, from which the subject power of attorney will result. 4) JEDZ (in accordance with the instruction from Annex No. 2 to the SWZ), appropriate as of the date of submission of tenders, for the purpose of temporary confirmation that the Contractor is not excluded from the procedure on the basis of art. 108(1) and art. 109(1)(4) of the Pzp Act and meets the participation conditions in the procedure specified in the SWZ. In the case of joint application for the contract by Contractors, JEDZ is submitted by each of the Contractors jointly applying for the contract; 5) JEDZ concerning other entities, on whose resources the Contractor relies, for the purpose of temporary confirmation, appropriate as of the date of submission of tenders, of the absence of grounds for exclusion on the basis of art. 108(1) and art. 109(1)(4) of the Pzp Act and meeting the participation conditions in the procedure specified in the SWZ, to the extent to which the Contractor relies on their resources; 6) a statement by the Contractor, current as of the date of submission of tenders, for the purpose of confirming that the Contractor is not excluded from the procedure on the basis of art. 5k of Regulation (EU) 2025/2033 and art. 7(1) of the Act on special solutions in the field of counteracting the support of aggression against Ukraine and serving to protect national security, in accordance with the template constituting Annex No. 8 to the SWZ. In the case of joint application for the contract by Contractors, the statement is submitted by each of the Contractors jointly applying for the contract; 7) a statement by another entity, on whose resources the Contractor relies, submitted for the purpose of confirming, current as of the date of submission of tenders, that the entity is not excluded from the procedure on the basis of art. 5k of Regulation (EU) 2025/2033 and art. 7(1) of the Act on special solutions in the field of counteracting the support of aggression against Ukraine and serving to protect national security, in accordance with the template constituting Annex No. 8 to the SWZ; 8) in the case where the Contractor relies on the capacities or situation of entities providing resources: evidence confirming that the Contractor, in implementing the contract, will have the necessary resources of other entities, which in particular constitutes the commitment of these entities to make the necessary resources available to the Contractor for the purpose of implementing the contract or another documentary evidence confirming that the Contractor, in implementing the contracts, will have the necessary resources of these entities. 9) a statement by Contractors jointly applying for the award of the contract, from which it follows which deliveries or services will be performed by individual Contractors, in accordance with the template constituting Annex No. 7 to the SWZ (if applicable); V.
