Poland – IT services: consulting, software development, Internet and support – Świadczenie Usługi Serwisu Urządzeń wraz z Oprogramowaniem będących w posiadaniu Ministerstwa Zdrowia

Acting on behalf of and for the Ministry of Health, based on the granted power of attorney, the e-Health Centre with its registered office in Warsaw (postal code 00-184), ul. Stanisława Dubois 5A, announces a public procurement procedure, the subject of which is the provision of the Service for the Maintenance of Equipment along with Software owned by the Contracting Authority, i.e. the Ministry of Health in Warsaw at ul. Miodowa 15.

2. The contract has been divided into six parts: Part I – Actidata equipment, Part II - Cisco equipment, Part III - Dell equipment, Part IV - Fortinet equipment, Part V - HPE equipment, Part VI - IBM equipment.

3. A detailed description of the subject of the contract is set out in Annex 1 to the SWZ - Description of the subject of the contract and Annex 2 to the SWZ - Draft contract terms.

4. The contractor is obligated to implement the contract on the principles and conditions described in the Draft contract terms constituting Annex 2 to the SWZ.
